Band 6 Clinical Nurse Specialist - Fracture Liaison Service/Bone health

This is a fixed-term/secondment opportunity for 12 months to cover maternity leave.

An exciting opportunity has arisen within the Fracture Liaison Service (FLS) for a 12 month CNS fixed-term/secondment role.

The FLS works with patients over the age of 50 sustaining a fragility fracture to identify those with Osteoporosis/at high risk of fractures and placing them on a personalised treatment pathway.

Fragility fractures account for a significant number of hospital beds and NHS budget each year. The role of the FLS is crucial in reducing these figures and helping our patients to enjoy quality of life well into maturity.

We work with specialist teams including T&O, Orthogeriatrics & primary care. The successful candidate will work closely with patients, GP Practices and specialist services within the Berkshire area to promote bone health and Osteoporosis knowledge.

We are looking for a colleague who enjoys working as part of a small but well-established team and has experience of autonomous working. A passion for preventative health care is a must. Experience working in T&O or Rheumatology would be strongly desirable.

Main duties of the job

The role requires covering clinics and patient calls to provide results and involves extensive written communication. Experience of history taking and carrying out patient assessments would be desirable.

Duties include:

Working to identify patients sustaining fragility fractures and arranges comprehensive further investigations. Following results, recommending an appropriate treatment pathway to these patients.

Participating in local clinical audits and national collaboration for FLS.

Providing specialist education and training for staff, patients and primary care colleagues.

Participating in workshops for newly diagnosed patients.

Working to identify, assess and implement treatment pathways for patients independently.

Visiting wards, fracture clinics, day units and participates in Rheumatology clinics to assess patients within the best time-frame.

The post holder displays excellent communication skills- both written and verbally.

Person Specification


Experience

* Evidence of recent clinical practice relevant to the post (Trauma and Orthopaedics/Rheumatology)
* Significant post-registration experience at Band 5 level
* Registered Nurse/AHP
* Sound knowledge of osteoporosis and the role of fracture prevention
* Experience advising patients of recommended treatment or lifestyle changes
* Experience of history taking


Skills

* Excellent verbal and written English
* Excellent interpersonal skills with particular ability to liaise effectively with multidisciplinary team members
* Experience in Audits/Quality imrprovement
* Experience of both team working and Independent work
* Confidence providing education to trust colleagues, patients and primary care teams
