An exciting opportunity has arisen to join the Transitions Team. We operate a hybrid working model, with a requirement to be in the office two days per week. Successful candidates will support a citywide caseload of 17–25-year-olds with learning disabilities, physical disabilities and autism as they move into adulthood.
The team is a joint service between Birmingham City Council and Birmingham Children's Trust, providing statutory support for young people leaving education or care who have ongoing support needs. We are looking for innovative, passionate colleagues who put young people at the centre of their work. The role is both challenging and rewarding, and you will receive strong supervisory support.
You should be organised, flexible and self‑motivated, with excellent communication skills and the ability to work independently while making defensible, person‑centred decisions. You will need to apply the 3‑Conversation Model and the Care Act 2014, promoting choice, independence, resilience and wellbeing. Strong partnership working with internal and external professionals is essential, as is experience of the social worker role.
You must also be able to demonstrate:
* Knowledge of the Continuing Health Care process and appeals
* Skills in Section 117 aftercare planning
* Experience managing professional disputes
* Registration with Social Work England
* A commitment to equity, diversity and inclusion
