Job Summary

Clayton Medical Centre is seeking a part time Practice Nurse to join our friendly, family Practice to deliver high quality nursing care.

Clayton Medical Centre serves around 6,000 patients in and around Clayton-le-Moors. We pride ourselves on being a friendly welcoming team, providing a good quality, caring services to our patients and a range of standard NHS and local enhanced services.


About Us

The Practice team currently consists of: 3 GP Partners, 3 Salaried GP, 1 Advanced Nurse Practitioner, 2 Practice Nurses, 1 Assistant Practitioner and 2 Healthcare Assistants. Supporting our Clinical Team are our Senior Administrator, Reception Supervisor, 4 care navigators, 2 Administrators and 1 Prescription Administrator. We are a training practice and regularly welcome FY2, GPST and student nurses as part of their training rotation.

Job Description & Responsibilities

Key Responsibilities

* Support the Infection Control Lead and work to the IPC policy.
* Undertake holistic patient assessments with patient and their families.
* Support long term condition management including involvement with quality improvement and assurance initiatives. Teaching patients and families how to manage their condition.
* Support the management of care for frail and housebound patients.
* Support public health screening programmes including immunisation, vaccination, cervical screening and provide opportunistic health promotion advice.
* Support population health management targeting health inequalities and improving access.
* Provide routine nursing care in accordance with clinical based evidence, NICE and NSF.
* Participate in clinical audit and research projects including supporting development and updating of practice protocols and guidelines.
* Work within national and local protocols.
* Recognise boundaries of practice and know when and to whom patients should be referred.
* Support delivery of enhanced services and other service requirements.
* Be aware of responsibilities under current safeguarding of adults and children policies and adhere to policies and procedures.
* Participate in learning from complaints, incidents and near miss events.
* Undertake all mandatory training and contribute to clinical governance.
* Provide basic wound care in line with local enhanced services.
* Maintain accurate clinical records using data entry templates where available ensuring SNOMED codes are used effectively.
* Contribute to practice targets including QOF and Local Enhanced services.
* Maintain a clean, tidy and effective work area at all times.
* Chaperone patients when requested by GPs, Locums or other nursing staff.
* Support the team with clinical emergencies.
* Liaise with external services to ensure that patients are supported appropriately (vulnerable patients, etc).

You may on occasion be required to carry out other tasks dependent on factors including workload and staffing levels. You may be asked to extend your working day to cover for an absent colleague at short notice.


Person Specification

Experience

Essential

* Experience working autonomously
* Knowledge of infection prevention and control measures
* Experience managing elderly and/or housebound patients

Desirable

* 2 years post-registration
* Experience with chronic disease management
* Experience of quality initiatives
* An appreciation of the NHS landscape including relationships between the practice, PCN and ICB
* Working with EMIS Web

Qualifications

Essential

* RGN Qualification
* Current NMC Registration
* Diploma or certificate in long term condition management
* Child and adult vaccination certification

Desirable

* Cervical Cytology Qualification
* Family planning (women's health) qualification
* Mentor qualification
* Previous general practice experience

Skills, behaviours and Knowledge

* IT skills (MS Office, internet etc)
* Good clinical system knowledge (ability to record accurate clinical notes)
* Good communication and interpersonal skills
