Critical Care Outreach Sister/Charge Nurse (XR07)
An exciting opportunity has arisen to join the Critical Care Outreach Team as a Senior Sister / Charge Nurse. The post holder will be experienced in the field of Critical Care.
The successful candidate will be allocated to take charge of shifts, where they will coordinate the safe running of the team and workload.
This includes cross site working, lone working and providing ongoing support and leadership to junior Critical Care Outreach team members.
The successful candidate will provide advanced clinical assessment of the deteriorating adult patient in a variety of clinical areas throughout the Trust, deliver education to ward-based teams on the recognition and management of the deteriorating adult patient and support the Deputy Matron and Deteriorating Patient Lead to implement Trust wide quality improvement initiatives.

Main duties of the job
Therole provides extensive clinical advice, education and support to
wardbased multi-disciplinary teams across a variety of clinical areas
throughoutthe trust.
Thepost holder will work as an accountable, autonomous practitioner
toprovide comprehensive advanced assessment and care of the
deteriorating/acutelyill adult patient leading to early intervention and
treatment.
Thepost holder will work in partnership with the Allied Healthcare
ProfessionalsTeam to initiate appropriate care to prevent further
physiologicaldeterioration, identifying and referring those patients
whorequire admission to critical care.
Thepost holder will act as a clinical role model, promoting quality
evidence-basedpractice. Multi-disciplinary team liaison, education
andtraining in the care and management of the deteriorating/acutely
illadult will be key responsibilities.

Qualifications
Registered Nurse (Level 1).
Evidence of theoretical, educational and clinical development related to thefield of critical care.
Recognised teaching/assessing qualification.
National Outreach Framework Enhanced Competenciesachieved or in progress.
Educated to post graduate degree level or equivalent experience.
Advanced Life Support provider.
It is desirable that the post holder, hold a non-medicalprescribingqualification. Following attainment, the post holder willprescribe within their scope of practice for the management of thedeteriorating adult.Experience
Significant Critical Care Outreach experience at sister/charge nurse level.

Have knowledge of pathophysiology, anatomy and physiology in order to assess,plan, implement and evaluate treatment of the critically ill patient. Able tomake expert clinical judgements involving complex situations which requiresanalysis and interpretation to decide on the best course of action/treatment.
Competently and confidently care for the deteriorating/acutely ill adultutilising expert critical care skills and knowledge in a variety of clinicalsettings with indirect supervision as part of the multi-disciplinary team
Provide rapid response and support for patients identified by the NationalEarly Warning Score.
Undertake a comprehensive advanced patient assessment based on history takingand clinical examination and implement a management/treatment plan.
Provide direct patient care and support at the bedside until the patient isstabilised or transferred to an area with a higher level of care. Initiatetherapeutic interventions for example to manage haemodynamic and respiratoryinstability.
Refer to other members of the MDT and other specialities as appropriate.
Participate in the stabilisation and transfer of critically ill patients toan appropriate environment.
Competent in advanced clinical skills such as venepuncture, cannulation,arterial stabs, tracheostomy decannulation, airway management andcardiopulmonary resuscitation.
Accurately request pathology and radiological investigations and makeclinical decisions based upon the results.
Utilise highly developed communication and negotiation skills to deal withchallenging/conflict situations ensuring patient centred care.
Work autonomously taking responsibility for own decision making, referring tothe appropriate physician/ICU Consultant/Outreach Team Leader when advice andfurther clinical input is needed or when care and treatment goes beyondnormal service parameters or scope of professional practice.
Carry the crash bleep and respond to 2222 calls. Where required, carry thecritical care bed managers bleep out of
hours,liaising with the sisters/charge nurses to co-ordinate flow throughoutcritical care.
Respect patients rights to participate in decisions about their care and beable to advise them on complex and personal matters.
Communicate highly sensitive information to patients and their relatives,showing empathy and providing psychological support.
Maintain up to date clinical records in accordance with the most up-to-dateNMC and Trust guidance.

Deliver and participate in education programmes across the trust to enableward staff to care for the deteriorating /acutely unwell adult.

Maintain annual mandatory training requirements. Keep up-to-date withdevelopments, advancements within critical care by working clinically in thecritical care environment.
