Overview
We are looking for an enthusiastic and dependable Healthcare Assistant (HCA) to join our friendly team at Clayton Brook Surgery. The role is 24 hours per week, ideally Monday, Wednesday and Friday. The successful candidate will be expected to work at our partner practice, Moss Side Medical Centre in Leyland on occasions, so candidate mobility is required.
The successful candidate will support our clinical team to deliver high-quality, patient-centred care. You will assist with health checks, phlebotomy, ECGs, long-term condition monitoring, vaccination support, and health promotion activities. Experience in General Practice is essential, and confidence using EMIS Web would be an advantage. This role offers the opportunity to contribute directly to improving patient outcomes, supporting our QOF and Long-Term Conditions (LTC) LES targets, and making a difference within the local community.

Main duties of the job
* Support the clinical team in delivering high-quality, patient-centred care across both sites.
* Undertake routine observations (blood pressure, pulse, temperature, height, weight, BMI).
* Perform phlebotomy and ECGs;
* Conduct urinalysis and assist with long-term condition reviews (frailty, respiratory, cardiovascular, diabetes, hypertension).
* Record key QOF indicators (smoking status, alcohol intake, BMI, blood pressure) and assist with flu and B12 injections.
* Carry out NHS Health Checks, frailty assessments, and lifestyle screening.
* Provide basic wound care (if trained); deliver health promotion advice and encourage self-care and prevention.
* Ensure accurate clinical coding in EMIS Web; maintain infection control and CQC standards; support QOF/LES achievement through documentation.
* Work as part of a team with flexibility and commitment to continuous learning through meetings, audits and mandatory training.

Clayton Brook Surgery is a established GP practice based in Clayton Brook. We work closely with a partner practice Moss Side Medical Centre in Leyland to provide high-quality, patient-centred healthcare to our local community.
We have GP Partners, Salaried GPs, 1 Advanced Nurse Practitioner, Practice Nurse, Nurse Associate, HCA, Medicines Co-ordinator, a Business Manager, Practice Manager and an Operations Manager, plus Receptionist/Administrators. We are Training Practices with GP Trainers/Educators.

Job Title: Health Care Assistant

Responsibilities:
* Health checks
* ECG recording
* Blood pressure monitoring
* Supporting patients with Long Term Conditions
* Weight management
* Pre-diabetic checks
* Processing and management of laboratory samples requested by GPs/Nurses
* Vaccine/Cold chain storage, monitoring and recording
* Surgical equipment and vaccine re-stocking and stock rotation
* Ordering of clinical stock and restocking consulting rooms
* Preparing and maintaining environments and equipment before, during and after patient care interventions including assisting GPs during minor operations
* Assisting in the assessment and surveillance of patients’ health and well-being
* Assisting with the collection and collation of data on needs related to health and well-being
* Administering vaccines/injections (Influenza, Shingles, Pneumococcal, RSV and B12) under the supervision of the Practice Nurse in accordance with practice protocol

Person Specification
Qualifications
* Experience working within General Practice
* Use of EMIS Web
* Good understanding of quality and performance frameworks such as QOF, LTC LES, and other locally commissioned Enhanced Services
Experience
* Ability to provide quality care with good interpersonal, organizational and teamwork skills; effective time management; able to work independently following instructions; adaptable to change; motivated and flexible.
* Interest in continuous quality improvement and training others
Additional Qualifications
* GCSEs in English and Maths (A-C). Level 2 Diploma in Health and Social Care (or equivalent). Care Certificate: Skills for Care, Skills for Health and Health Education England. Demonstrable competency in Chronic Disease Reviews. Phlebotomy qualification
* Qualified to administer flu vaccines, B12, RSV, Shingles and other vaccinations
