An exciting opportunity has arisen to join the Home First Team in Buckinghamshire.
The team is still in its infancy of becoming a substantive service, providing team members with the opportunity to influence and shape its development.
We are seeking a skilled and motivated registered health or social care professional (Physiotherapist, Occupational Therapist, Nurse, or Social Worker) to support patients recently discharged from hospital, helping them regain independence and remain at home.
The ideal candidate will possess excellent communication skills, experience in leading teams, developing services, quality improvement, and data monitoring. They should be approachable, supportive, and capable of offering advice and making decisions in the best interests of patients.
Knowledge of discharge processes and pathways, along with the ability to interpret and relay health and social care information accurately, is essential. Building and maintaining effective relationships with professionals and stakeholders is also crucial. Experience in both acute and community settings is advantageous.
The role involves working remotely, on-site, and visiting patients at home as required.
Responsibilities include:
1. Providing proactive, personalized, multidisciplinary case management for discharged patients.
2. Coordinating safe and effective discharges supporting ongoing care and assessment at home.
3. Working with patients and therapy staff to set, review, and update recovery goals using a strength-based approach.
4. Utilizing personal expertise and team skills to improve outcomes for patients and carers.
5. Collaborating with Home Care agencies to facilitate reablement and ensure appropriate care packages.
6. Supporting timely discharge while ensuring necessary reablement/rehabilitation interventions.
7. Addressing care gaps through flexible planning, signposting, referrals, and interventions.
8. Documenting information for sharing and monitoring service performance.
Follow the agreed case management pathway to ensure process consistency.
