Adult Social Worker - Community Wellbeing Team £31.00 per hour East Yorkshire 37 hours p/w, hybrid Agency contract About the role: We are looking to appoint an agency Social Worker to support Community Wellbeing Teams across East Riding of Yorkshire Council, initially supporting the Wolds Community Wellbeing Team and Holderness Community Wellbeing Team. There is an expectation of flexibility to travel across the area. The role will involve undertaking: Care and Support Assessments Care and Support Planning Reviews Mental Capacity Assessments and Best Interest decisions Safeguarding enquiries (Section 42) Supporting hospital discharges where appropriate Visits will need to be completed face to face, with the option of hybrid working. What you need: A degree in Social Work or equivalent Registered with Social Work England At least 3 years PQE direct employment within English Local Authorities Completed relevant post-qualifying training Enhanced DBS check Excellent communication and written skills Why Work with Pertemps Social Care?
