Job Summary
We are looking for an enthusiastic and capable individual who can provide support in the delivery of the medicines optimisation service provided to GP practices in Stockport. As a PCN Lead Pharmacist you will support our service by ensuring delivery of good quality Medicines Optimisation and cost effective quality prescribing to patients in your PCN. You will lead a team of Practice Support Pharmacists, GP Pharmacy Technicians and Medicines Co-ordinators. Applicants must meet the essential criteria in the person specification. They should also have significant experience within Primary Care settings. This is a part-time (30 hours per week) role, and is fixed-term through the end of October 2026.

Job Responsibilities
* Provide pharmaceutical support to a designated group of GP practices to enable them to develop, implement and monitor an agreed prescribing plan.
* Assist practices/care settings in developing formularies, guidelines and policies to implement NICE guidelines and other prescribing requirements.
* Advise on risk management relating to prescribing and monitoring of specialist drugs.
* Review prescribing areas using data (e-PACT) and identify areas for action.
* Carry out structured medication reviews and liaise with carers and care staff to optimise therapy and reduce waste.
* Develop and review practice policies on repeat prescribing and reinforce prescribing guidance.
* Liaise with Community Pharmacists to support prescribing changes and identify potential savings.
* Respond to medicine-related queries from GPs, staff, PALS, social care and the public.
* Provide disease register improvements and training for staff on medicines usage.
* Participate in multidisciplinary meetings and training related to medicines usage.
* Attend interface meetings to improve cost-effective and evidence-based medicine use and build links with hospital pharmacists.
Responsibilities for HR
* Supervise, direct and monitor the work of Practice Support Pharmacists, Pharmacy Technicians and medicines coordinators.
* Support development of GP Practice and Medicines Optimisation staff via leadership and guidance.
* Identify personal and team training needs and maintain CPD requirements.
Responsibilities for Physical and Financial Resources
* Promote rational and cost-effective prescribing at practice and health economy levels.
* Analyse prescribing data (e-PACT) to manage budgets and advise on cost-effective actions.
* Monitor QOF/DES/IIF/QIPP targets and communicate progress to practices.
* Ensure correct use and security of equipment and data.
Communications and Leadership
* Explain complex information clearly to diverse audiences.
* Provide written reports and patient-facing information on therapeutic topics.
* Demonstrate tact and negotiation to drive change and build relationships across the network.
* Maintain confidentiality of patient data and monitor industry guidance to counter misinformation.
Policy and Service Development
* Contribute to medicines management policy development and implementation.
* Support prescribing policy development in practices and care settings.
* Evaluate work for effectiveness and potential replication in other practices.
Qualifications
Essential
* Vocational Masters Degree in Pharmacy, 4 years (or equivalent).
* One year pre-registration training.
* Registration with the General Pharmaceutical Council (GPhC).
* Evidence of commitment to Continuing Professional Development.
Desirable
* Post-graduate Diploma in clinical pharmacy or equivalent.
* MRPharmS or other registrations (e.g., CPPE PCPEP, Independent prescriber).
Knowledge, Skills and Attributes
* Knowledge of the wider NHS and multiple clinical areas including chronic disease management.
* Excellent written and oral communication and interpersonal skills.
* Persuasive negotiating and influencing abilities; ability to instigate and manage change.
* Strong organisation, planning, teamwork and the ability to work under pressure.
* Willingness to travel between practices.
* Computer literate; high level of clinical reasoning.
