To be part of the Urgent Community Response Team / Virtual Ward that provides a seven-day-a-week, rapid assessment and intervention service for adults of all ages who do not require specialist acute hospital admission but need crisis intervention within a community setting. This may be due to changes in physical or psychological well-being, or temporary social circumstances making community maintenance difficult without short-term rehabilitation or intervention.

Virtual wards deliver acute clinical care at home for a short duration as an alternative to hospital care, utilizing a step up/down approach. Patients in a virtual ward have their care reviewed daily by a Geriatrician or Advanced Clinical Practitioner via a digital platform that enables remote monitoring and escalation to a multidisciplinary team.

The post holder will participate in a seven-day rota, including weekends and public holidays. Responsibilities include conducting assessments, determining care levels, initiating care plans to ensure patient safety, and supporting the virtual element of the service by delivering appropriate care within patients' homes.

* Develop blurred boundary competencies (Nursing/therapy appropriate) to assist the MDT in managing ongoing clinical risks.
* Manage referrals into the Urgent Community Response Team/Virtual Ward, liaising and coordinating staff, and managing service capacity.
* Provide short-term interventions to maintain patient safety at home or prevent re-admission, such as providing equipment or domiciliary care, or managing ongoing care (Geriatrician-led).
* Work autonomously within Trust policies and professional standards, maintaining accurate and comprehensive up-to-date records.
* Determine clinical diagnoses, develop, deliver, evaluate, and modify individualised treatment plans, and provide specialist advice, ensuring appropriate care pathways and communication through liaison and referral to other agencies as needed.
