We are looking for a Diabetes Specialist Nurse to join our supportive and innovative Diabetes Team at The Rotherham NHS Foundation Trust. This is an exciting opportunity to work within a multidisciplinary service that delivers evidence-based care to people with complex Type 1 and Type 2 diabetes across community and acute settings.
What Were Looking For
* Registered Nurse with current NMC registration.
* Post-registration experience in diabetes care, ideally in a community or specialist setting.
* Excellent communication, assessment, and decision-making skills.
* Passion for delivering patient-centred care and promoting independence.
* Ability to travel independently across the Rotherham area Ifnecessary, adjustments can be considered in accordance with the Equality Act2010.
A postgraduate qualification in diabetes or working towards one is desirable, but we are also committed to supporting professional development for the right candidate.
Main duties of the job
As a Diabetes Specialist Nurse, you will
* Provide expert clinical care and self-management support to people living with diabetes.
* Deliver structured education and individualised care planning in line with national guidelines.
* Work autonomously and as part of a multidisciplinary team including dietitians,podiatrists, GPs, and hospital-based diabetes teams.
* Support the upskilling of other health professionals through training and education
* Take part in service development and quality improvement initiatives.
