We are excited to announce the recruitment for an Occupational Therapy post within the Hospital at Home Team to support our Urgent 2-hour Community Response (UCR) service and virtual ward offer, to best meet the needs of our local population. The Hospital at Home Service and is an exciting place to work, with many training opportunities available.
This is a 7-day service and operates from 08:00-20:00, with a base at Custom House, Ellesmere Port.
You would be expected to be able to cover all aspects of the shift pattern. This is a full time role, 37.5 hours a week.
You will be a member of a holistic team which consists of GPs, Advanced Clinical Practitioners, Registered Nurses, Therapists, Therapy Assistants, Pharmacists and Clerical Support.

Main duties of the job
Provide a 2-hour response for patients entering a health crisis and provide therapy input to patients on the virtual wards.
Work as an autonomous practitioner to perform a specialist therapeutic assessment of patients with diverse presentations. Use clinical reasoning to provide a diagnosis, deliver and adapt an individualised treatment programme within the patient's own home.
Work in collaboration with the Community Care Teams, Northwest Ambulance Service, Social Services, Millbrook and the patient's registered GP, to support the patient at home.
Work flexibly in response to service needs, ensuring allocation of an appropriate caseload for the Therapy Assistants.

Qualifications
Essential criteria
* Diploma/Degree in Occupational Therapy
* Evidence of relevant postgraduate courses
* Fieldwork educator’s qualification
* HCPC registration
Desirable criteria
* Member of Royal College of Occupational Therapy (RCOT)
Knowledge and Expertise
Essential criteria
* Knowledge of Occupational Therapy specific theoretical frames of reference in line with evidence-based practice
* Professional guidelines and codes of conduct
* Critical appraisal

* Evidence of CPD and portfolio demonstrating interest in a specialist area
