Overview

Carlisle Healthcare PCN is a 2-practice PCN comprising Carlisle Healthcare and Spencer Street Surgery with a population of around 54,000 patients. We are recruiting a newly qualified GP who is within the first two years of completing their CCT. The postholder must not have had a substantive role in the practice at the time of application.

The GP will undertake a variety of duties across both practices within the PCN and within the visiting team, including face-to-face consultations, phone consultations, triage, home visits to housebound and care home patients, and related paperwork such as labs, prescriptions and correspondence. The role requires autonomous work and collaboration within a multi-disciplinary team, providing medical services to the PCN population under the remit of the GMS and PCN contracts. Adherence to GMC standards for good medical practice and a positive, collaborative working relationship within the team are expected.

Main duties include delivering GP services across both practices, which may involve in-house surgeries or home visiting, and linking with the PCN Visiting team.

Carlisle Healthcare PCN comprises Spencer Street Surgery and Carlisle Healthcare in Carlisle, with a joint population of around 54,000 patients. We work on areas such as care of patients who are housebound or living in care homes. The PCN employs GP, Paramedics, ACPs, Nurses, Care Co-ordinators, Social Prescribers, Pharmacists, Physiotherapists and Occupational Therapists.

Our PCN delivers care to around 54,000 patients across 5 sites. We aim to provide exemplary patient care and to develop innovative general practice solutions. The team includes GP, Clinical Pharmacists, Social Prescribers, Paramedics, First Contact Physiotherapists, Advanced Practitioners, Nurses, Care Co-ordinators and Occupational Therapists.


Job Description

ARRS GENERAL PRACTITIONER

SESSIONS

Main Duties and Responsibilities

Clinical responsibilities include: working across the rota to undertake surgery consultations, telephone consultations and queries, home visits, checking and signing/releasing repeat prescriptions, and dealing with paperwork and correspondence promptly. Make professional autonomous decisions, assess undifferentiated problems, screen for disease risk factors, provide counselling and health education, admit/discharge patients as appropriate, and maintain clear contemporaneous notes. Complete computer-generated prescriptions in line with formulary, and visit patients during surgery time in emergencies when clinically indicated.

Confidentiality and data protection are essential. Training/meetings, patient management, and the development of practice protocols are included. The post holder will be expected to travel across the network and work flexibly to meet business needs.

Supervision and governance duties include recording data for audits, contributing to practice audits, and maintaining professional standards. Infection control and safety procedures must be observed, and the post holder will support continuous learning and development.


Person Specification

Experience – Essential: experience in urgent care and/or primary care; dealing with a range of clinical conditions; providing home visits; ability to evaluate the safety and effectiveness of own practice; understanding of evidence-based practice; ability to work autonomously with some supervision. Desirable: experience in triage; knowledge of Quality and Outcomes Framework; knowledge of local public health issues; awareness of broader health issues; knowledge of health promotion strategies.

Other Skills and Attributes – Excellent communication (written and oral), strong IT skills (MS Office, Outlook), EMIS Web experience, time management, problem solving, ability to work autonomously and in a multidisciplinary team, empathy, adherence to policies, confidentiality, and continuous professional development.

Knowledge and Skills – Ability to triage, assess and diagnose, request and interpret pathology tests, manage acute deteriorations of long-term conditions, evidence-based practice, patient risk management, governance knowledge, and appropriate referrals within scope of practice.

Qualifications – Essential: Newly qualified GP with CCT within 2 years and full GMC registration and license to practice; experience of working in a GP training practice using EMIS Web.
