What is D2A?
The D2A team is therapy‑led, operational 7 days a week since 2021, and works alongside social care colleagues in START (Short Term Assessment & Reablement Team) and a range of other services. Together, we support early discharge from hospital, enabling patient assessment in their own homes with a rehabilitation focus. The goal is to maximise safety and independence in daily living activities through focused, goal‑oriented assessments that promote self‑management and overall wellbeing.
This role is part of a wider expansion supported by Scottish Government funding to enhance Moray’s delivery of the NHS Grampian Home First and Unscheduled Care Improvement programmes.

What You’ll Need
* A qualification in Occupational Therapy
* HCPC registration
* A full UK driving licence
