Overview
Urgent Care / Intermediate Care Clinical Nurse. Permanent, full-time role within the integrated community team. Works across Urgent Care, Intermediate Care, and occasionally supports the Rehab Unit. Based primarily at Union House, Torquay, with community visits across Torbay. A full driving licence and access to a car are essential.
The role includes supervisory and delegation responsibilities, supporting and guiding non-registered colleagues as part of the wider team. You will plan and implement safe, effective care, coordinating immediate home support or a place of safety bed in consultation with the IC/Duty Team.

Responsibilities
* Undertake holistic, comprehensive assessments including risk assessment and nursing assessments of the patient’s medical, social, and emotional state to determine the most appropriate plan of intervention
* Provide ongoing assessment of nursing needs of patients and carers in the community; plan care with appropriate staff delegation
* Work as an autonomous practitioner in planning and implementing nursing treatment programmes
* Advise and instruct carers and patients on health and well-being, and self-care strategies
* Carry out specialist and shared complex nursing assessments, including continuing and palliative care
* Develop patient-centered, goal-oriented plans of treatment and/or rehabilitation to avoid unnecessary admission or to facilitate early discharge
Qualifications and Skills
* Qualified Registered Nurse
* Evidence of continuing professional development
* Diverse post-registration experience
* Desirable: BSC (Hons) in Community Health Nursing (District) or equivalent
* Nurse prescribing (or willingness to undertake training); team management skills
* Comprehensive understanding of health care services; training in frailty and long-term condition management
* Knowledge and understanding in frailty and long-term condition management
* Nursing mentor
* Essential: broad range of clinical skills including complex and specialist skills
* Ability to communicate complex information; maintain timely and accurate records
* Prioritise workload and delegate appropriately
* Holistic client assessment across a wide range of procedures; sound clinical judgment in complex or crisis situations
* Autonomous practitioner; ability to manage a caseload
* IT knowledge/experience; awareness of budget management; good understanding of clinical governance
* Desirable: awareness of local and government agendas; experience in acute care (secondary and community care)
Specific Skills
* Assessment, planning, implementing and evaluating care of the unwell adult
* Knowledge of Research and Audit principles
* Negotiating skills; Health and Safety/Risk Assessments
* Current driving licence and access to transport
* Acute care nursing skills; ability to coordinate care for people with complex needs
* Teaching and presentation skills
* Desirable: Cannulation and IV administration; Palliative care nursing skills
