PCN Clinical Pharmacist
This role is pivotal to improving quality of care and operational efficiencies which requires motivation and passion to deliver an excellent service.
The role will involve working within the Primary Care Network and our practices.
If you hold an MPharm qualification and have registration with the GPhC this could be the role for you.
Applicants will need to complete the CPPE pathway - 18 months, which you would be enrolled on once in post.
Responsibilities
* Support general practice staff with prescription and medication-related queries.
* Manage the repeat prescription process and handle acute prescription requests.
* Carry out medicines reconciliation after care transitions and promote safer prescribing.
* Provide expert clinical advice while considering healthcare and social care needs of patients across the practice.
* Engage in Structured Medication Reviews (SMR) and Long-term conditions (LTCs) management.
* Support elderly and frail patients and care home medication reviews as appropriate.
* Lead on person-centred Medicines Optimisation and contribute to Quality Improvement Prescribing Indicators (QOF).
* Lead on local Anti-Microbial Stewardship (AMR) within the PCN and work with Pharmacy teams across Primary Care, Community and Hospital to improve patient outcomes.
* Contribute to Enhanced Services and QOF, and participate in the wider healthcare team as part of service delivery.

Required and Desirable Qualifications
Experience Desirable
* Minimum 2-years post-registration experience in Primary Care Pharmacy or in any other sector of Pharmacy
Qualifications
* MPharm
* Mandatory GPhC registration
* Independent Prescribing Qualification (or currently working towards)
Desirable
* Post-graduate clinical pharmacy qualification (e.g. clinical diploma, MSc, other post-graduate clinical modules) or equivalent experience
