We are offering an exciting and unique new role as a Band 4 Health Care Support Worker within our Integrated Mental Health Teams (IMHT) across South Staffordshire, supporting our new First Transitions Pathway.

The First Transitions Pathway is a new service to help support individuals with complex mental health as they move from the structured support of forensic services to the community-focused care offered by our Integrated Mental Health Teams. Their focus will be on building rapport, implementing effective engagement strategies, and facilitating access to community resources.

As part of this role you will be expected to:

* Provide intensive and assertive outreach to service users at risk of disengagement, aiming to prevent relapse.
* Provide practical support within community settings, addressing immediate needs and implementing engagement strategies.
* Be curious and motivated to make a difference to this cohort of individuals.
