Birmingham Community Healthcare NHS Foundation Trust (BCHC) is recruiting Band 5 Community Public Health Nurses to support our Early Years and Health Visiting teams as part of our integrated service for children and families in Birmingham.

This is an exciting opportunity to help develop and implement an integrated way of working with our children’s centre partners. We are looking for Band 5 staff nurses who are passionate about delivering high-quality services while embracing innovation and creative thinking to support our Health Visiting and Early Years team.

Responsibilities

* Undertake health needs assessments and screening for children as directed by the named or delegating Health Visitor, ensuring prompt feedback and escalation of concerns as required.
* Participate in planning, delivery and evaluation of health promotion activities as part of an integrated team for individuals, families, groups and communities using evidence-based practice.
* Contribute to the BFS early help offer, supporting families to access additional support as required in line with the early help framework.
* Contribute to safeguarding of children at risk in line with safeguarding policies and procedures.
* Participate in and promote childhood immunisation programs as appropriate.
* Participate in the Trust staff nurse development program and other related Trust-based learning opportunities.
* Work within the NMC code of professional conduct.
* Support Health Visiting Teams in managing children with additional medical needs, completion of Educational & Health care plans, and preparing/supporting families for transition to education.
* Deliver evidence-based packages of care for children in line with BFS SOPs for Health Visiting and clinical pathways (e.g., premature care pathways and children with Special Educational Needs).
* Liaise with Multi-Disciplinary Teams to ensure effective communication and coordination of care.
* Support Early Help intervention and complete Early Help Plans as required.
* Maintain accurate and contemporaneous electronic patient records in line with NMC requirements and Trust policies.
* Convey complex information to other professionals and clients with well-developed oral communication skills, including negotiation, empathy and reassurance to gain cooperation and overcome barriers.
* Undertake clinical audits as directed and support action plans arising from findings.
* Manage difficult situations, report concerns, and support families through challenging circumstances such as illness and bereavement.
* Plan, organise and run clinics to support the health and developmental needs of children as required.
* Deliver care in a variety of settings (children’s centres, clinics, GP surgeries and clients’ homes) which may require lone working in community settings.
* Maintain professional knowledge and skills; attend mandatory and relevant update training as required by the Trust.
* Co-operate with research and survey programmes relating to the client group and participate in teaching/training of staff and learners where required.


Qualifications and Requirements

* Band 5 Community Public Health Nurse with registration in good standing (NMC).
* Commitment to delivering high-quality services, with a collaborative, integrated approach to working with Health Visiting and Early Years teams.
* Ability to work within safeguarding policies and procedures and within the NMC code of professional conduct.
* Excellent communication and interpersonal skills, with the ability to work with families and other professionals.
