HomeCareDirect is looking for an experienced and compassionate Registered Paediatric Nurse to join our team as a Regional Clinical Lead. This is a home and community-based role where you’ll work autonomously, managing your own caseload of children and young people with complex health needs across the Nottingham area. Other areas will be considered as our service for children is being rolled out nationwide.
You will play a key part in helping the people we support stay well, safe, and out of hospital, while ensuring their lives are fulfilling and aligned with their personal aspirations.
About the Role
As a key clinical lead, you will support individuals and their families to live independently in line with their care and support plans. You will work closely with personal assistants and the wider HomeCareDirect team to deliver exceptional, person-centred care.
You will conduct regular client visits, complete support plans, carry out clinical assessments, lead PA inductions and supervisions, and ensure all care provided meets professional and regulatory standards.
Key Responsibilities
* Support individuals and families in line with their care plans, promoting independence and problem-solving with a positive, proactive approach
* Ensure clients receive high-quality care at home and that their needs are consistently met
* Carry out regular client visits to review care, undertake risk assessments,conduct PA observations and supervisions, and complete annual support reviews
* Meet prospective clients (at home or in hospital) to discuss how HomeCareDirect can support them
* Develop personalised care and support plans in partnership with the individual, family members, and MDT professionals
* Assess PA training needs and deliver training aligned with your clinical competencies
* Work collaboratively with our Regional Care Team and Head Office to maintain health, safety, and compliance standards
* Undertake inductions, and supervisions for PAs in partnership with clients
* Ensure all documentation is completed accurately and within expected timeframes
* Attend MDT meetings and support clients as outlined in their support plans
Requirements
What We’re Looking For
Essential experience and qualities:
* Qualified Paediatric Nurse (minimum Band 5) with a current NMC registration (PIN required)
* Experience in complex care and working independently in community settings
* Strong care-planning and risk assessment skills
* Confident using Microsoft Office, with access to the internet to use our online care planning system
* Ability to train PA’s within your clinical competency scope
* Full UK driving licence
* Compassionate, supportive, and able to build strong relationships with individuals, families, and PAs
* Excellent communication skills, with a professional and non-judgemental approach
* High attention to detail and commitment to quality and compliance
* Flexible and open to changes aligned with service needs
* Must live in or near Nottingham, or be willing to travel

* Salary of £44,850, rising to £48,750 after a successful 6-month probation
* Mileage paid at 48p per mile (first 10,000 miles), then 25p per mile
