We are looking for a Clinical Pharmacist to join our Pharmacy Team. The role is full time, but we would consider a job share for the right candidates.

Main duties of the job
To be able work as part of a multi-disciplinaryteam in a patient-facing role.
To take responsibility for areas of chronic diseasemanagement within the practice and undertake clinical medication reviews toproactively manage patients with complex polypharmacy.
To provide primary support to general practicestaff with regards to prescription and medication queries.
To support the repeat prescription system, dealwith acute prescription requests, and medicines reconciliation on transfer ofcare and systems for safer prescribing, providing expertise in clinicalmedicines advice while addressing both public and social care needs of patientin the GP practice(s).

Job responsibilities
* See, whereappropriate, patients with single or multiple medical problems where medicineoptimisation is required
* Review the on-goingneed for each medicine, a review of monitoring needs and an opportunity tosupport patients with their medicines taking ensuring they get the best use oftheir medicines i.e. medicines optimisation
* Undertake structuredmedication reviews /medication review with patients and produce recommendations
* Work with care homestaff to improve safety of medicines ordering and administration.
* Signposting tocommunity pharmacy and referring to GPs or other healthcare professionals whereappropriate
* Answers relevantmedicine-related enquiries from GPs, other practice staff, other healthcareteams, e.g. community pharmacy, and patients with queries about medicines.Suggesting and recommending solutions. Providing follow up for patients tomonitor the effect of any changes
* To reconcilemedicines following discharge from hospitals, intermediate care and into carehomes, including identifying and rectifying unexplained changes and workingwith patients and community pharmacists to ensure patients receive themedicines they need post discharge
* Ensure patients haveappropriate monitoring tests in place when required
* Identification ofcohorts of patients at high risk of harm from medicines, this might includerisks that are patient related, medicine related, or both
* Contributepharmaceutical advice for the development and implementation of new servicesthat have medicinal components (e.g. advice on treatment pathways and patientinformation leaflets)
* Analyse, interpretand present medicines data to highlight issues and risks to support decision-making
* Undertake clinicalaudits of prescribing
* Implement changes tomedicines that result from MHRA alerts, product withdrawal and other local andnational guidance
* Monitor practiceprescribing against local and national guidance
* Auditing practicescompliance against NICE technology assessment guidance
* Provide newslettersor bulletins on important prescribing messages
* Provide education andtraining to primary healthcare team on therapeutics and medicines optimisation
* Work with the generalpractice team to ensure the practice is compliant with CQC standards wheremedicines are involved

Person Specification
Skills and Knowledge
* Recognises priorities when problem solving and identifies deviations from the normal pattern and can refer to seniors or GPs when appropriate
* Able to obtain and analyse complex technical information
* Produce timely and informative reports
* Gain acceptance for recommendations and influence/motivate/ persuade the audience to comply with the recommendations/ can plan, manage, monitor, advise and review general medicine optimisation issues in core areas for long term conditions agreed course of action where there may be significant barriers
* Can plan, manage, monitor, advise and review general medicine optimisation issues in core areas for long term conditions
Experience
* Minimum of 2 years post- qualification experience
* An appreciation of the nature of GPs and general practices
* An appreciation of the nature of primary care prescribing, concepts of rational prescribing and strategies for improving prescribing
* In depth therapeutic and clinical knowledge and understanding of the principles of evidence- based healthcare
Qualifications
* Registration with the General Pharmaceutical Council
* Master's degree in pharmacy (MPharm) (or equivalent)
* Independent prescriber
* Specialist knowledge acquired through post-graduate diploma in clinical pharmacy or equivalent training/experience
* Membership of the Royal Pharmaceutical Society
* A member of or working towards Faculty membership of the Royal Pharmaceutical Society
* Completion of Centre for Pharmacy Postgraduate Education (CPPE)- Primary Care Pharmacy Education Pathway or MSc Advanced Practice Pathway.
