What the Job Involves The position entails delivering personalized and responsive care assessments, case management, and planning to support users and caregivers. As part of the Transition Service, you will collaborate with the Children with Disabilities Team, Leaving Care Team, and SEN to provide crucial support. Key responsibilities include facilitating EHCPs, health action planning, and Continuing Health Care assessments, aiding young people in employment opportunities, and working in partnership with various statutory and voluntary organizations. The role also involves undertaking assessments related to the Mental Capacity Act, and making necessary referrals and safeguarding enquiries. Skills Timely and responsive care assessment and planning Strong case management abilities Experience with EHCPs and health action planning Proficiency in conducting Mental Capacity Act assessments Ability to liaise and partner with various agencies Knowledge of safeguarding and risk assessment procedures
