The post holder will support the colorectal nursing team in providing a colorectal service that is evidence based and provides seamless care following agreed pathways. The role will include assessment of patients with colorectal conditions and the development of individualized management plans in conjunction with the colorectal surgeons. Support Enhance Recovery Program and new colorectal cancer patients in Oncology Clinic. The provision of education to the colorectal patient is essential and the post holder will have excellent communication skills. The post holder will possess a colorectal nursing/stoma care qualification or have relevant experience in this area of nursing. Post holders will act in a manner that upholds the values of the Trust and that of the profession ensuring the service delivered contributes to supporting the principles of clinical governance.
Responsibilities
* Under the direction of the Colorectal Nurse Specialists and guidance of the Colorectal Surgical team to promote early supported discharge of CRC patients, discharging patients safely and assisting in reducing lengths of stay adhering to national guidelines.
* The post holder should ensure the needs of all colorectal patients are recognised and met by the assessment, planning, implementation and evaluation of care.
* To participate in MDT with the colorectal team and promote care management to ensure the patients journey is seamless.
* Act as a point of liaison for MDT and multi‑agency working in order to achieve the best possible outcome for patient care.
* Advice and support to patients undergoing colorectal/stoma care surgery.
Teaching & Development
* To participate in teaching programmes to educate and develop other staff/patients and carers on an ongoing basis on the wards as required. To ensure the sharing of clinical and theoretical knowledge to promote research based practice.
* Maintain the Trusts aims and objectives within the clinical setting.
* Accept accountability and responsibility for accurate and legible record keeping in accordance with local policies and NMC guidelines.
* Undertake additional training as required to meet the needs of the patients and register any additional skills acquired as a result of enhanced practice.
