This is an exciting opportunity for an experienced Registered Mental health nurse looking to join the South Gwynedd community mental health team.

You will have excellent communication skills, be self motivated, proactive and committed to the promotion of innovation for service delivery.

The successful candidate will hold a complex caseload of adult individuals with severe and enduring mental health issues and will work collaboratively to assess the needs and create care & treatment plans to meet the identified needs and risks identified.

The post holder will be required to work independently and liaise with a variety of agencies and third sector organisations plus the individual's carers and family members.

You will have exceptional communication skills and the ability to build and maintain effective working relationships, demonstrating a 'can do' attitude'.

The ability to speak Welsh is desirable for this post; Welsh and/or English speakers are equally welcome to apply.


Main duties of the job

Promote positive health and work towards the prevention of physical and psychological ill health.

Work as part of a multi-disciplinary team, directing, encouraging and leading therapies ensuring that they are delivered effectively.

Participate in the duty/crisis rota for the South Gwynedd Mental Health Services, carrying out specific duties as outlined and processing team referrals, advising and directing to appropriate services.

Be responsible for providing accurate assessment of referred cases to determine the suitability for continued intervention.

Prepare for and contribute to appropriate clinical team and other multi professional meetings

Monitor and administer prescribed psychotropic medication, liaising closely with prescribers with due regard to assessed effectiveness and the management of adverse effects.

Be conversant with Mental Health Law and apply this knowledge to nursing practice, and oversee its application by junior staff.

Attend and contribute to relevant meetings and reviews, legal hearings and case conferences, providing written and verbal reports.

In addition to BCU mandatory training the post holder must undertake specific job related mandatory training as documented in the CPG Training Plan.

Work flexible hours including occasionally outside office hours on a planned basis in order to meet service needs.

Person Specification


Experience

* NMC Registered Nurse (Part 3 or 13)
* Sufficient post registration experience. Experience within a relevant clinical area
* Evidence of good organisational skills Evidence of basic I.T. skills Evidence of people management and leadership skills. Demonstrate the ability to care and communicate an advanced understanding of the psychological needs of individuals. Ability to undertake comprehensive assessment of service users needs along with the ability to provide therapy and treatment. Evidence of analytical skills Effective problem solver Advanced clinical skills.
* Up to date knowledge and application of consent, data protection, patient confidentiality and risk management with Health and Safety issues. Up to date working knowledge of the Mental Health Act. Up to date working knowledge of the Mental Health Measure 2010 Knowledge of national strategies related to sphere of nursing. Knowledge of Information and Technology Communication Knowledge of the principles of change management.
* Committed to continuous professional development Fitness and willingness to use de-escalation and where required safe holding, breakaway techniques (as appropriate) subject to relevant training according to BCUHB Policy. Ability to travel between sites in a timely manner
* Diploma or degree in relevant clinical / professional subject
* Experience of resource management. Experience of policy development.
* Knowledge of Interview and Appraisal process. Knowledge of management theories. Knowledge of local policies / procedures and guidelines.
* Ability to effect change in a positive manner.


Skills

* Be a team player Act as a positive role model by influencing and sharing skills and knowledge Highly motivated and with the ability to motivate and influence others. Ability to use own initiative in complex situations.
