School Nurse

South Tyneside 0-19 Team have an exciting opportunity for ahighly motivated and enthusiasticSCPHN trained School Nurse, who wants to make a real difference improvingoutcomes for children and families. We are looking for candidates whoare highly motivated and possess excellent communication and leadership skillsto join us.

The successfulcandidate should be wellorganised, have excellent interpersonal skills and be able to communicateeffectively with children and young people both in school and within othereducation provider settings. You must be committed to working effectively both withina team and independently and should possess a knowledge of the current trendsand developments within school nursing and the health agenda. Main duties of the job

Follow policies and procedures of South Tyneside and SunderlandFoundation NHS Trust

The identification of Health needs within a locality corporate caseloadand deliver appropriate support and care of children and their families.

Responding to health needs assessments by referring to otherprofessionals & other agencies.

Assess, organise, plan and implement and evaluation of screeningprogrammes and national health promotion guidelines.

Identify families with safeguarding issues and support these familiesthrough the safeguarding process.

Undertake programmes of care in partnership with parents/carers &other agencies.

Facilitate health assessments and plan care for children.

Identify children with disabilities and facilitate support and referralsto maximise developmental potential. Act as a key worker when indicated,ensuring effective, efficient and appropriate care plans.

Provide accessible support to parents on an individual or group basis.To include child development, behaviour management, social & environmentalfactors that affect health, safety, and public health issues.

Initiate, participate in and evaluate health promotion activities.

Job responsibilities

To work unsupervised as senior school nurse withcaseload responsibility to influence the health and wellbeing of childrenand families to improve the life chances and give every child the Best Start inLife

To be responsible for leading the Healthy ChildProgramme and screening through home visiting, clinics and multiagency settingse.g. Schools and Childrens Centres

To lead on CHAT Health

To work with a wide range of partner agencies tosupport children and families in need, looked after children and children wherethere are child protection concerns

To be the lead for assessment, early identification andcare planning for vulnerable families, children with additional needs and theimplementation of targeted specialist programmes of care e.g. health &lifestyle choices, continence and Solihull behaviour management

To lead on and deliver public health sessions inEducations settings including sexual health, smoking, emotional wellbeing andother key public health messages

To lead on and deliver Teacher training in Schools

Work in collaboration with local statutory,voluntary and mainstream services to ensure that children and families canaccess other health and support services

To be responsible and accountable for the deliveryof safe, effective and compassionate care

To always work to the NMC Code and own competencies

To supervise and support non-registered staff asstipulated by the NMC under the requirements of delegation

To work with children, young people and families ina non judgemental manner in order to promote health outcomes

Person Specification


Qualifications

* Specialist Community Public Health Practitioner School Nursing
* Teaching qualification
* Nurse Prescriber


Skills and Knowledge

* Excellent communication and interpersonal skills to manage discussions with families and other agencies, write complex and sensitive reports e.g. child protection
* Able to work effectively as part of a team
* Able to assess. Plan, implement, supervise and evaluate service delivery
* Able to work effectively with a range of other multiagency professionals to ensure best outcomes for children
* IT skills to enable the production of reports, communicate via email, receive reports/communication
* Able to carry out audit of workload as required
* Resilience, self-awareness, non-judgmental, driven to improve outcomes for children
* Knowledge of current policy, legislation relating to children, families, public health
* Willing to undertake further training related to support role


Experience

* Experience of working in a community setting/public health role /health promotion
* Experience of working in a multiagency way
* Ability to organise and prioritise workload.
* Works to agreed standards and protocols


Physical skills

* Ability to assist clients to move as per Trust Moving and Handling Policy
* Carriage of equipment necessary for role
* Car driver with access to a car.
