Band 4 - Discharge Coordinator, Integrated Discharge Team

We are looking for a motivated individual to work within the Integrated Discharge Team at Watford General Hospital.
The Discharge Coordinator is a Band 4 position full time hours at 37.5 PW. This is a secondment for 10 months. This also includes covering a 7 day service.

Main duties of the job
The Discharge Coordinator will act as the point of contact for the ward teams, AHP's, IDT and SPOC promoting an effective and smooth discharge for all patients on their designated wards.
They will be responsible for highlighting any referrals which the DPN will need to complete. They will then ensure that all aspects of the patient's discharge is progressed and escalate when there are changes or delays to the EDD, patient needs or clinical condition.

Job responsibilities
A key part of the role will be to ensure there is early communication with families/carers/care homes to ensure all relevant information is collated and potential barriers are highlighted so they can be addressed and solved prior to when the patient leaves hospital and mitigate failed discharge.
Alongside these elements the post holder will be responsible for ensuring accurate data is captured on all data bases in order to fulfil all Trust reporting requirements.
Person Specification
Education and qualifications
* Evidence of a good standard of general education/GCSE or equivalent - Maths and English level c or above or equivalent
* Basic IT skills
* Evidence of ability to undertake job related training.
* Willingness to undertake training to meet competency framework.
* Evidence of relevant short course and in-service training.
* To have completed recent care certificate or be committed to complete on employment.
* Knowledge of specific patient conditions, types of therapy intervention acquired through training and experience.
* NVQ Level 3, health or social care qualification or equivalent
Knowledge
* Evidence of knowledge of Hospital Environment and Pathways
* Good knowledge of Health and Safety legislation and safe working practices.
* Accepts responsibility for own workload.
* Recognises the limits of own authority within the role
* Seeks and uses support appropriately
* Understands and practices the principles of confidentiality at all times.
* Knowledge of related legal frameworks for social and health care.
* Knowledge of local health and social care services.
* Experience of working with health/social care databases
* Understanding of how to prioritise patients in need of the service
Experience
* Experience of undertaking of administration work in a hospital or other healthcare setting
* Related experience of working in a rehabilitation/Reablement or healthcare environment with older people.
