We have an amazing opportunity for two experienced nurse to join our world leading team at St Bartholomew’s Hospital. The post holders will deliver high-quality nursing care to patients with structural heart disease as well as patients with acute coronary syndrome.

To have excellent communication with patients, their families, colleagues, and other members of the internal and external multidisciplinary team.

To support the team coordinating MDT’s for the structural team and the acute coronary care team and enacting the MDT plans

To work collaboratively in Consultant-led clinics

To be a member of the ward-based Structural and ACS teams; reviewing patients pre and post intervention and providing support to nursing staff.

Be an active member of the valve multidisciplinary/ACS team meeting; advocating for patients.

To undertake regular teaching, training, and mentorship of the nursing team in the provision of care for Structural heart disease patients/ACS.

To actively participate in and promote research and service improvement and development.

The post holder will be based at Barts Heart Centre

Main duties of the job

The MDT coordinator’s post is to provide dedicated support to the structural cardiology multi-disciplinary teams (“MDT”). The post holder will be expected to ensure the MDT runs smoothly and to time. Post holder will be required to live type at the meeting, which will be signed off by MDT Lead. They need to ensure all patients are discussed within agreed timescales, and treatment plans are actioned post MDT meeting. Post holder will use specialist knowledge of the patient pathway to proactively influence the tracking records using high levels of communication to expedite appointments/procedures as necessary and to resolve issues which could lead to delays in treatment.

Person specification

Qualifications

Essential criteria

1. Registered Nurse (Degree level or equivalent)
2. Relevant post graduate cardiac course
3. Evidence of continued professional development

Desirable criteria

4. Possession of or working towards Masters
5. Physical assessment course or willing to work towards when in post

Experience

Essential criteria

6. Extensive post registration experience
7. Extensive experience working AFC Band 6 within cardiology.
8. Experience of looking after patients with heart valve disease
9. Experience of patient education

Desirable criteria

10. Previous experience of working as a Clinical Nurse Specialist.
11. Research and audit experience
12. Clinical and operational management experience.

Knowledge

Essential criteria

13. Expert Knowledge of the current guidelines for the management and treatment options of heart valve disease.

Desirable criteria

14. Knowledge of the inpatient and out-patient referral systems.

Skills

Essential criteria

15. Excellent communication and interpersonal skills with MDT, patients, and their careers
16. Ability to work as part of team as well as autonomously without supervision
17. Ability to prioritise and organise work to manage a caseload of patients.
18. Proven oral and written communication skills. Demonstrates managerial, organisational and leadership skills.
19. IT skills, data input, collection, analysis, audit, evaluation, and use of EXCEL
20. Ability to deal with sensitive and confidential information

Desirable criteria

21. Previous experience of writing standard operating protocols
22. Ability to manage change.
23. Ability to digest and interpret verbal instruction effectively
