Lincolnshire Community Health Service (LCHS) invites Registered Nurses to join the Grantham Community Nursing Team. Positions available include one full‑time fixed term role and one part‑time fixed term role.
Responsibilities
* Assess care needs, plan, deliver and evaluate nursing care using evidence‑based practice.
* Provide wound management, continence care, palliative care, medication administration and long‑term condition management.
* Work autonomously and collaboratively with multidisciplinary teams (Social Workers, GPs and Allied Health Professionals) to coordinate patient care.
* Deliver services in patients' homes, care homes and clinic settings, fostering therapeutic relationships and person‑centred care.
* Participate in a 12‑week induction programme, preceptorship and clinical skills programme offered by LCHS.
Requirements
* Valid registration as a Registered Nurse with NHS.
* Availability to travel to patients' homes and clinic settings; access to suitable transport is essential due to rural coverage.
Working Hours
* Early shift: 08:00–16:00
* Mid shift: 09:00–17:00
* Late shift: 10:30–18:30
* Weekend shifts: 08:00–17:00 or 08:00–18:30
