Overview
Provide health assessment, support and care coordination for children and young people in care aged 0-25 years in St Helens.
Key Responsibilities
* Maintain a caseload of Children in Care, producing comprehensive health assessments and developing individualized care plans.
* Identify and manage health risks, collaborating with health and social care colleagues to provide inclusive primary health services.
* Facilitate health education and health promotion to children, carers and partner agencies.
* Deliver training and support to Children in Care teams and external stakeholders.
* Ensure effective information sharing for children leaving care and capture the wishes, feelings and voice of each child.
* Attend meetings and contribute to service improvement initiatives.
Qualifications & Experience
* Registered nurse with experience in Child and Adolescent Health or comparable health setting.
* Strong leadership and caseload management skills with experience of working with vulnerable children and young people.
* Excellent assessment, risk management and communication skills.
* Willingness to travel and flexible approach to working across schools, homes and other settings.
