Retain Healthcare is developing a new model of care to support people living at home in your community. This is an opportunity to shape and develop a new service and deliver care that improves quality of life for those in need.
Main Duties and Responsibilities:
* Coordinate with Social Workers, District Nurses, and other professionals within the multidisciplinary team.
* Communicate changes in requirements effectively.
* Adhere to company policies and procedures.
* Attend training sessions for personal development.
* Assist clients with personal needs such as dressing, washing, meal prep, and community access, as per the Care Plan.
* Perform light housework tasks like dusting, vacuuming, cleaning, ironing, and meal preparation when working in clients' homes.
* Encourage clients to undertake personal care and tasks they are capable of, recognizing their changing abilities.
