Leaving Care (Positive Futures) - Social Worker Pay rate - £30.15 - 38.76 Swindon Borough Council The role is within in our Leaving Care Service where you will manage a caseload of young people who are preparing to leave care. You will also be working with the allocated Pathway Advisors for the young person who are also based within our service. Our service links in early with the young people transferring for other services and you will lead on the completion of pathway plan assessments and the initial pathway plan. It will remain your ongoing responsibility to maintain the plans in line with the required legislation up until the young person is 18 years old when they will then fully transfer to their allocated Pathway Advisor. In an addition to managing your own caseload you will also be required to be part of our duty system that provides additional support to our young people alongside our Pathway Advisors.
