Ormskirk, West Lancashire Full-Time | £25-£40 per hour | Monday–Friday, 9am–5pm Band 6 | Community Mental Health | Locum Opportunity We are currently seeking a skilled and experienced Band 6 Mental Health Practitioner to join a supportive and dynamic Community Mental Health Team in the West Lancashire area. This is a fantastic opportunity for an experienced clinician who is confident working in fast-paced, community-based settings and can contribute to high-quality care delivery. Key Responsibilities: Hold and manage a caseload of service users with a range of complex mental health conditions Complete comprehensive mental health assessments in both the community and clinical base Develop, implement, and review individualised care plans tailored to patient needs Conduct risk assessments and risk management planning, ensuring appropriate interventions Work collaboratively within a multidisciplinary team including social workers, psychologists, psychiatrists, and support staff Support service users’ recovery and social inclusion through person-centred and trauma-informed approaches Liaise effectively with other healthcare professionals, families, and community services to ensure continuity of care Requirements: Qualified Mental Health Professional (e.g., RMN, Social Worker, Occupational Therapist) Active NMC/HCPC registration Demonstrated experience of working with individuals with complex mental health needs in a community setting Strong knowledge of risk assessment and care planning frameworks Ability to work autonomously and collaboratively within a team Full UK driving licence and access to a vehicle – community visits are essential Why Join Us?
