We are expanding and have an exciting opportunity for an experienced home carer to take on a rewarding role that truly makes a difference to people living in their own homes across South Cheshire.
We are recruiting for a Care Assessor to support our Registered Manager in ensuring the care we provide is person‑centred and of the highest quality.
Responsibilities
* Conduct full care needs assessments for new clients in their own homes.
* Work with the Team Leader and Care Coordinator to ensure client call times are appropriate for their needs while balancing the home carers rosters.
* Carry out introductory visits with new clients.
* Develop person‑centred support plans and risk assessments in line with client requirements and changing care needs.
* Review and update support plans regularly, working with your Team Leader to ensure all assessments are completed within our set timescales and the client monitoring system is updated accordingly and shared with the care team.
* Monitor care quality through spot checks, client feedback, and regular reviews.
* Assist with matching care staff to clients to allow professional relationships to develop and deliver the best care possible.
* Provide guidance and support to care staff regarding support plans and client requirements.
* Update the care team with any changes required, working closely with your Team Leader and liaising with a variety of other healthcare professionals to provide holistic support to each person we support.
* Provide support to the care team by being part of the On Call rota during the week and weekends.
* The Care Assessor role is 60% field‑based, with time spent completing client care calls so that you get to know each client and their needs, and is flexible according to the number of new client assessments and client reviews.
This role is 60% field based and allows you to fully understand the care calls, where times can be altered, and increases your knowledge to assist with On Call support.
You will need to be available for mornings - 7am until
2.30pm and evenings 4pm until 10pm.
