An exciting opportunity has arisen for a Band 6 Community Respiratory / Oxygen Practitioner to join our supportive and forward-thinking team in Mid Powys.

This community-based role offers the chance to work autonomously while being part of a wider multidisciplinary team, delivering high-quality care to patients with chronic respiratory conditions, including those requiring oxygen therapy.

You will manage a varied caseload across clinic and home settings, using your specialist skills to support proactive care, improve patient outcomes and promote self-management.

We would particularly welcome applications from Welsh speakers; but even if you don't speak any Welsh, or want to develop your skills, don't worry: the health board will support you with training.


Main duties of the job

* Assess, plan, implement and evaluate care for patients with chronic respiratory conditions, including those requiring oxygen therapy
* Manage a defined caseload, providing specialist respiratory and oxygen assessment, treatment, review and follow-up
* Work in partnership with patients, carers and the multidisciplinary team to deliver person-centred care and support self-management
* Provide education, advice and clinical expertise to patients, carers and colleagues
* Undertake non-medical prescribing (where qualified) in line with local and national guidance
* Work autonomously, prioritising workload, making clinical decisions and managing risk within scope of practice
* Support coordinated care across health and social care, including admission avoidance and discharge planning
* Contribute to service development, audit, quality improvement and implementation of evidence-based practice
* Maintain accurate patient records and support governance and safeguarding responsibilities


Job responsibilities

The post holder will provide specialist assessment, care planning and management for patients with chronic respiratory conditions, including those requiring oxygen therapy, across community settings in Mid Powys. Working autonomously and as part of a multidisciplinary team, the practitioner will manage a defined caseload, delivering person-centered care through clinics and home visits. The role involves undertaking comprehensive assessment, implementing and evaluating care, initiating appropriate treatments (including non-medical prescribing where qualified), and supporting safe and effective management of patients in line with local and national guidelines.

In addition, the post holder will provide expert advice, education and support to patients, carers and colleagues, promoting self-management, informed decision-making and health prevention. They will contribute to coordinated care across health and social care systems, supporting admission avoidance, discharge planning and complex care needs within the community. The role also includes involvement in service development, audit and quality improvement, maintaining accurate clinical records, supporting governance and safeguarding responsibilities, and contributing to education, mentorship and the ongoing development of the respiratory service.


Person Specification

* Live registration with NMC or relevant professional body (e.g. HCPC)
* Degree in a health-related subject or equivalent experience
* Specialist respiratory knowledge/qualification or equivalent experience
* Management or leadership qualification or experience
* Experience of education, mentorship or supervision of staff or students
* Experience working within community or integrated care settings
* oExperience of proactive caseload management of wide range of patients including those with chronic and respiratory conditions
* oSignificant clinical experience in assessing, planning, implementing and evaluating care for the patient group of the caseload
* oKnowledge of current Nursing/Therapy issues within NHS care systems
* oUse computer systems to a well-developed level - email, word processing, spreadsheets
* oWell-developed verbal and written communication skills oEffective multi-agency team player, including the ability to engage positively with carers
* oMotivate, enthuse and encourage innovation within the team through effective communication skills
* oKnowledge and skills to manage patients with complex needs
* Management or leadership qualification or experience
