Specialist Nurse Looked After Children and Care Leavers

We are seeking an enthusiastic and innovative Band 6 Specialist Looked After Children and Care Leaver Nurse to deliver high‑quality, compassionate care. The role involves leading statutory health assessments and identifying, addressing, and advocating for the health needs of children in care and care leavers.
You will support a vulnerable group of children and young people with often complex physical, mental health, and emotional wellbeing needs, working to improve health outcomes and promote healthy lifestyles.
In this role, you will support the Named Nurse in delivering high‑quality, compassionate, and child‑centred care for Looked After Children and Care Leavers. You will take a lead role in undertaking statutory health assessments, managing your own caseload, and developing and reviewing holistic care plans to meet the complex physical, emotional, and mental health needs of children and young people.
You will work collaboratively with a wide range of multi‑agency partners, including social care, education, and other health professionals, attending and contributing to meetings to ensure coordinated and effective care. The role also involves providing specialist advice, guidance, and delivering training to colleagues, carers, and partner agencies to promote best practice and improve health outcomes.
You will contribute to service development, policy implementation, and quality improvement initiatives, ensuring care is evidence‑based and aligned with local and national guidance. Job responsibilities
The post holder will support the Named Nurse for Looked After Children in delivering a high‑quality, effective, and compassionate health service for Looked After Children, Care Leavers, and other vulnerable groups. They will contribute to improving health outcomes by ensuring statutory requirements are met and that children and young people receive timely, holistic, and child‑centred care.
The role includes undertaking statutory health assessments in a variety of settings, including home, clinic, and community environments, and developing, implementing, and reviewing individualised health care plans. The post holder will manage their own caseload, ensuring effective prioritisation and coordination of care in partnership with children, young people, carers, and professionals.
The post holder will work collaboratively within a multi‑agency framework, attending and contributing to professional, strategy, and review meetings, and liaising with colleagues across health, social care, education, and other partner organisations. They will provide specialist advice, guidance, and support on the health needs of Looked After Children and Care Leavers, including complex and sensitive cases.
A key element of the role is to promote evidence‑based practice and contribute to the development, implementation, and review of policies, procedures, and clinical guidelines in line with local and national standards. The post holder will support safeguarding practices and ensure that children and young people remain at the centre of all decision‑making.
The role also involves delivering and supporting training for health professionals, foster carers, and partner agencies, helping to raise awareness and improve knowledge of the health needs of Looked After Children. The post holder will contribute to service development, quality improvement initiatives, audit, and performance monitoring to ensure continuous improvement of the service.
In addition, the post holder will demonstrate effective team working, act as a positive role model, and support colleagues through supervision, advice, and shared learning. They will engage in continuous professional development, participate in appraisal and supervision processes, and maintain up‑to‑date knowledge in line with professional and organisational requirements.

* 2 years post registration as a Registered Nurse (Child, Adult, Learning Disability, or Mental Health) with the Nursing and Midwifery Council (NMC).
* Health Visiting or School Nursing professional qualification with entry onto the third part of the NMC register.
* Evidence of post‑graduate training in relevant topics.
* Evidence of Level 3 Safeguarding Children.
