An exciting opportunity has arisen for Administrators to work within the Somerset Hub for Coordinating Care team. Due to an exciting change to the service, training will be provided, and the role will be based in Wells at Priory House during induction, then in Ashford Court, Taunton.
The Somerset Hub for Coordinating Care includes the Intermediate Care Bed Hub, Somerset Primary Link Urgent Referral Service, Hospital at Home Remote Monitoring Service, and End of Life Care Coordination Centre. We are a friendly, hardworking team with a strong focus on health and wellbeing.
This role will primarily be based within the Somerset Primary Link Team, but successful applicants will have opportunities to work across different teams.
Candidates should have strong attention to detail, advanced multitasking skills, and be comfortable working in a busy environment.
Our service hours are 8:00 AM to 9:30 PM, seven days a week, including bank holidays, and the service operates year-round.
Main duties of the job
You will work closely with clinicians in the Somerset Hub for Coordinating Care to facilitate smooth referral processes for service users and patients, acting as a single point of contact for patient transfer and admission to community hospital beds within the Primary Care Trust.
Additional responsibilities include supporting clinicians with referrals, booking transport, liaising with patients, working across multiple computer systems, coordinating telephone calls, and completing reports.
