Integrated Transfer of Care Co-ordinator

ECCH is proud to be partnering with our local ICS to introduce a new Transfer of Care Co-ordinator role in our Transfer of Care team.

The Co-ordinator will need a range of experience, which may include hands-on care or similar experience in a Health and Social Care setting. The primary requirement is a passion for providing effective support to ensure individuals receive the best care.

As the co-ordinator, you will be at the heart of the team, supporting team members to be efficient, effective, and to achieve the best results for the people within the service.

Main duties of the job

1. Work closely with system partners from the ICB, ECCH, JPUH, SCC, and NCC to represent the five organisations working collaboratively on behalf of the GYW place.
2. Support safe transfers of care for patients in the Great Yarmouth and Waveney Locality (East Place).
3. Use the Home First ethos and effective communication to promote community patient flow with system partners.
4. Coordinate incoming referrals for community hospital beds and care, liaising with patients, families, primary care, and social care teams to transfer care appropriately.
5. Chair daily reviews of patients and contribute to daily board rounds for East Place Beds with Care, taking action to prevent delays and escalate concerns.
6. Support the operation of the service which runs 7 days a week, with shifts as required.

Job responsibilities

1. Communication: Review referrals for appropriateness, ensure all relevant information is complete, and signpost as needed. Interpret sensitive information and document accurately. Maintain effective relationships with multidisciplinary teams. Manage patient information and referral assessments. Understand transfer pathways and criteria. Liaise with operational hubs, social care, voluntary services, housing, and charities. Maximise patient flow and bed capacity. Participate in daily system calls and meetings. Support evidence for CQC standards. Manage patient flow from acute to community.
2. Professional: Review hospital waiting lists for appropriate referrals. Analyze dashboard data for trends. Develop effective relationships with system partners. Provide timely referral information. Facilitate early transfer of care. Work flexibly to meet demands. Act as a resource and role model. Demonstrate knowledge of relevant legislation. Use decision-making and problem-solving skills. Support process and policy development. Contribute to financial and resource efficiency.

Personal Attributes

Qualifications

* NVQ Level 4 in health and social care or equivalent
* ILM Level 3


Experience

* Experience in hospital and community multidisciplinary settings
* Experience in developing and implementing processes
* Knowledge of patient flow, The Care Act 2014, and NHS CHC


Skills and Knowledge

* Interpret complex patient information with sensitivity
* Diplomacy, teamwork, and ICT skills
* Attention to detail and documentation skills
* Analytical and problem-solving abilities
* Prioritization and workload management
* Assertiveness and conflict management skills
