Overview
The post holder will work as a member of the Overnight Nursing Service, providing direct and indirect patient care as directed by a trained member of the team to support patients and their families within their own home environment. The post holder will undertake clerical duties as directed, including electronic documentation, and work as an effective member of the multidisciplinary team. Due to lone working policy, the post holder will never work alone and will always be with a band 6 specialist nurse. Applicants must be able to travel independently across Stockport and have access to a suitable vehicle for business purposes. Adjustments can be considered in accordance with the Equality Act where possible.
Responsibilities
* Provide basic nursing, therapy and social care to people and carers, carrying out delegated clinical, therapeutic and personal care tasks.
* Promote the physical, mental and emotional health and wellbeing of individuals and client groups.
* Be an effective member of the multidisciplinary team.
* Communicate effectively with patients and carers, recognising needs for alternative methods of communication.
* Carry out care and other duties under direct supervision of a registered professional.
* Understand support services available in the community.
* Maintain excellent written patient records and adhere to patient confidentiality in accordance with Caldicott guidelines.
* Recognise and respond to challenging behaviour; maintain liaison with colleagues and communicate appropriately with other departments/services as required.
* Provide feedback regarding patients' conditions to appropriate health care professionals.
* Contribute to safeguarding and promoting the welfare of children, young people and vulnerable adults in line with Trust policies and procedures.
* Enhanced DBS check may be required at cost (£54.90); if successful, cost deducted from first Trust salary. The DBS update service may be required with annual subscription (£16).
