Within Aneurin Bevan University Health Board, our Clinical Futures Strategy sets out how we are moving to delivering care closer to home. Primary and Community Services are at the heart of the model and central to developing a new relationship with patients investing and strengthening community and social care services to support and treat patients in their homes and communities providing quality outcomes. In order to deliver the strategy two permanent positions have become available within the Bargoed and Pontllanfraith District Nursing Teams in Caerphilly locality.
Opportunities
* 1 WTE permanent Pontllanfraith District Nursing Team
* 1 WTE permanent Bargoed District Nursing Team
Responsibilities
* Provide cost effective, efficient and high quality patient care.
* Carry the continuing responsibility for the assessment, planning, implementation and evaluation of care needs and quality assurance.
* Line manage a nursing team, ensuring the professional and clinical development of team members.
* Promote evidence based practice and act as an accountable role model.
* Ensure that, together with their team members, care is delivered in accordance with the NMC Professional Code of Conduct and other relevant policies.
Qualifications and skills
* Ability to speak Welsh is desirable; Welsh and/or English speakers are equally welcome to apply.
