Overview

A Vacancy at University Hospitals Coventry and Warwickshire NHS Trust.

This is a fantastic opportunity for Adult and Paediatric qualified Advanced Clinical Practitioners to join our dynamic team of Advanced Clinical Practitioners in Emergency Medicine. The post holder will be working autonomously within the Emergency Medicine footprint with opportunities for continued support for professional development. We are looking for passionate individuals who are able to work autonomously using complex decision making skills to provide expert and excellent patient centred clinical care across our Urgent and Emergency care footprint utilising the four pillars of advanced practice; clinical practice, leadership and management, education and research. The postholder will be a qualified Adult or Paediatric Advanced Clinical Practitioner able to demonstrate advanced critical thinking in clinical decision making and be able to work collaboratively with the Emergency Medicine teams and be key to the shaping of the Advanced Clinical Practitioner service.

The post holder will have an MSc in Advanced Practice and be able to provide a portfolio of evidence demonstrating the Scope and Standard of their practice in Emergency Medicine, utilising NHSe capability frameworks and or competencies and capabilities as outlined in the RCEM credentialing document. Successful applicants will be required to provide an immunisation and vaccination report, from either their current UK occupational health provider or their UK GP.


Responsibilities

* Act as proactive clinical leader and a role model of expert clinical practice and deliver clinical care across the Urgent and Emergency care footprint according to local and national guidelines to ensure delivery of evidence based care.
* Provide advanced levels of practice, knowledge and skills to promote inspiration and development of the Advanced Clinical Practitioner role.
* Undertake mentorship, and clinical supervision of staff, assessing competence against set local, regional and national professional and clinical standards.
* Document clearly all aspects of assessment, diagnosis, and management plan, presenting findings using approved documentation framework.
* Participate and contribute to Advanced Practice and specialist forums, locally regionally and nationally.
* Critically appraise the outcomes of relevant research, evaluations and audit and disseminate evidence based practice and audit locally, regionally and nationally via relevant forums.
* The post holder will be required to work within all areas of the urgent and Emergency care footprint; Adult and Children\'s Emergency Departments, Acute Medicine areas and Urgent treatment centres, and be willing to support the development of the Advanced clinical Practitioner service across Emergency Medicine.
* Coordinate care across the patient pathway with the multidisciplinary team to ensure effective management of Emergency Medicine patients.
* Lead and deliver clinical care in a variety of settings in line with national and local guidelines, ensuring delivery of evidence-based care and working towards the Trust vision of providing a World Class Service.
* Undertake initial assessment to determine differential diagnosis, utilising advanced critical thinking and decision-making skills, and refer to senior clinical colleagues when necessary.
* Assess, prioritise and provide expert clinical care contributing to the patient\'s treatment plan to meet physical, psychological, and social care needs.
* Request and interpret relevant investigations to inform the development of the patient\'s management plan.
* Proactively refer to other clinicians or services as needed to deliver care beyond current scope of practice.
* Initiate treatment plans for patients requiring intervention and treatment with ethically based considerations.
* Identify and manage long-term condition risks where appropriate.
* Undertake relevant clinical procedures to facilitate patient care with appropriate training and supervision.
* Autonomously assess, diagnose, prescribe (as per scope of practice and regulatory requirements), and review medications for therapeutic effectiveness in acute/emergency and chronic illness contexts.
* Provide information and advice to patients on prescribed regimens, side-effects and interactions in line with evidence-based practice and local protocols.
* Integrate pharmacological and non-pharmacological treatments into patient management plans.
* Document all aspects of assessment, diagnosis, and management plan and present findings using the approved documentation framework.
* Maintain high-quality communication within a complex environment with competing priorities.
* Understand and apply safeguarding measures to identify vulnerable adults and children, and be aware of statutory procedures and local guidance.
* Undertake delegated consent as appropriate after successful completion of relevant training and competencies.
* Report clinical incidents via Trust Datix reporting system.


Communication

* Use the SBAR tool to communicate clinical concerns to the senior medical staff when a patient is critically unwell or deteriorating.
* Communicate complex information effectively to patients and carers, recognising diverse needs and preferred methods of communication.
* Maintain effective communication with the multidisciplinary team and other services, including providing constructive feedback.
* Demonstrate advanced communication skills in breaking bad news and supporting patients, family and staff.
* Anticipate barriers to communication and take action to improve it.
* Act as an advocate for patients and colleagues.
* Access sources of support (e.g., PALS) and information in an accessible format for all patients, referring appropriately.
