About Caremark Mansfield & Ashfield

We are on a journey of developing our complex care offering to supports children, younger people and adults in their own home environments who have complex conditions who live in the area. Our Care Supervisors have an allocated caseload of clients and associated staff teams. They engage closely with our clients, family members and professionals, they monitor, support and develop a dedicated staff team, guiding, motivating and inspiring them to be focussed on achieving their client’s goals and outcomes.

About the Role

Our Care Supervisor role is community based, it is important that the successful individual is organised, has strong planning skills and can operate on their own as well as being part of our well established team. We expect the successful individual to be highly visible in the community, visiting their assigned clients on a regular basis and being a pivotal part of the service that we provide. The pivotal element of the role is to inspire a team of Support Workers to enable our clients to reach their potential.

Our service extends to weekends and through the night support, as such flexibility is essential to ensure that the service is monitored and supported through regular staff spot checks, reviews and observations.

In addition to this, some of our clients have support at school and will need engagement through the day. Case Leaders work flexibly and plan their own diary in order to meet the requirements of the service whilst also maintaining a good work-life balance.

Our Case Leader will participate in an ‘on-call’ rota, which is the services emergency out of hours facility. This will be on a 1 week on, 1 week off rota basis.

Principal Responsibilities

1. Produce and maintain a person centred care plans for all new customers, ensuring all risk are assed and recorded approprialty.
2. Support and develop staff in designated packages of care through close supervision and monitoring on a daily or weekly basis to enable them to provide the standards of consistent care, delivered to each client
3. Engage with and support our staff team through Team Meetings, Supervisions, Spot Checks, Annual Reviews and Observations, including covert checks and night time and weekend checks
4. Conduct regular, cyclic reviews with clients, involving family members, professionals and advocates where appropriate
5. Monitor, audit and maintain records within the clients homes.
6. Introduce new staff members to their new clients, ensure staff are fully aware of the clients individual care plan and are focussed on helping the client to achieve their goals, outcomes and aspirations
7. Recognise positive comments and compliments provided by clients, family members and professionals and feed this information back to staff members through Connect Team, cards, team meetings and supervisions, to ensure they can be recognised
8. Conduct live audits and dynamic medication and record keeping audits in the client’s home to ensure best medication practice

Essential Role Requirements

9. Previous community complex care experience minimum of 3 years
10. Level 4 Diploma or above in Health and Social Care
11. Previous experience in leading a team or managing people
12. A full UK driving licence and own vehicle
