Minor Illness Nurse - Potterells Medical Centre
Weare seeking a Minor Illness Trained Independent Prescriber to join oursupportive team and deliver high quality, safe, patient-centred care.
As a MinorIllness Independent Prescriber you will assess, diagnose and manage patientspresenting with a variety of minor illnesses across the primary care setting.Your ability to prescribe independently will enable you to provide efficientand effective treatment, reducing waiting times and improving patient outcomes.
Ideally, we would like this to be a hybrid roleof minor illness sessions interspersed with practice nursing sessions coveringroutine nursing aspects.
We are ideally looking for 2.5 to 3 days a week on Tuesday, Thursday and Friday from 9am-4.30 or 5pm. Weekly hours would therefore range between 17 hours to 22.5 hours per week depending on shift preference.
Salary £24-£28 depending on experience.

MainDuties:
* -Conductthorough clinical assessments and formulate management plans for patients withminor illnesses.
* -Prescribemedications safely and responsibly within your scope of practice.
* -Workcollaboratively with GPs, nurses, pharmacists and other team members to deliverseamless care.
* -Providehealth promotion advice and support patient self-care.
* -Maintainaccurate clinical records and comply with all governance and regulatorystandards.
* -Hybridworking covering both Minor Illness and practice nursing and Long-TermCondition management where appropriate.

About Us: We are currently a 6 Partner practice based in Welham Green. We also have 5 Salaried GPs, 5 Nurses, 2 HCAs, 2 Pharmacists, a Pharmacy Technician as well as a range of ARRS staff and admin support. We cover 2 sites, and this role will work across both. Our main site is at Potterells Medical Centre in Welham Green, and we also cover Hertfordshire University with our second site being the Medical Centre on College Lane Campus.

Job responsibilities
As a Minor Illness practitioner, you will:
* Assess, diagnose, plan, implement and evaluate treatment/interventions and care for patients presenting with acute illnesses for children through to the elderly.
* Interpret and evaluate diagnostic test results in liaison with the patient and their GP and initiate treatment accordingly
* Prescribe as an independent nurse prescriber accordingly within the legal framework of nurse prescribing
The following are the core responsibilities of the Practice Nurse in delivering health services:
* Support the Infection Prevention Control (IPC) lead and work to the IPC Policy
* Consult with patients and their families to undertake assessments of patient need
* Support in the long-term condition management within the organisation, including being involved with quality improvement and assurance initiatives
* Support public health and screening programmes, including immunisations, vaccinations, and cervical screening and also provide opportunistic health promotion advice
* Embed population health management approaches within general practice, targeting health inequalities and improving access
* As required, provide routine nursing care to patients in accordance with clinical based evidence, NICE and the NSF
* Support new-to-practice nurses and other healthcare professionals within the organisation
* Teach and advise patients and their families on how to manage their condition
* Participate in clinical audits and research projects and implements changes as required, including supporting with the development, and updating of practice protocols/guidelines and procedures locally
* Work within national and local protocols where these exist
* Recognise the boundaries of their practice and know when and to whom patients should be referred
* Support in the delivery of enhanced services and other service requirements
* Be aware of duties and responsibilities regarding current legislation and adhere to practice policies and procedures on Safeguarding Adults and Safeguarding Children
* Participate in the management of patient complaints when requested to do so and participate in the identification of any necessary learning brought about through incidents and near-miss events
* Undertake all mandatory training and induction programmes
* Support the practice team with the reviewing and implementation of practice policies and protocols, ensuring conformance to extant legislation
* Attend a formal appraisal with their manager at least every 12 months. Once a performance/training objective has been set, progress will be reviewed on a regular basis so that new objectives can be agreed
* Provide wound care (ulcer/Doppler, etc.) to patients
* Maintain accurate clinical records in conjunction with extant legislation ensuring that codes are used effectively
* Contribute to practice targets (QOF, etc.), complying with relevant guidance
* Maintain a clean, tidy, effective working area at all times
* Chaperone patients where necessary
* Support the team in dealing with clinical emergencies
Person Specification
Experience
* - Broad knowledge of general practice and experience of working as a Practice or Community Nurse
* - Experience with chronic disease management
* - Experience of infection prevention control measures
Qualifications
* - Registered Nurse with NMC
* - Non Medical Prescribing Certificate
* - Qualifications in family planning and HRT desirable
