Band 5 Occupational Therapist – Discharge to Assess (D2A)
Location: Moray Health & Social Care Partnership

Overview
The D2A team is therapy‑led and operates 7 days a week, working alongside social care colleagues to support early discharge from hospital. You will conduct assessments in patients’ own homes, focusing on safety and independence through goal‑oriented rehabilitation.
Responsibilities
• Provide timely, evidence‑based OT assessments to enable discharge and self‑management
• Work collaboratively with the D2A, START (Short Term Assessment & Reablement Team) and other services
• Document and report outcomes to the multidisciplinary team
• Contribute to service improvement and quality assurance initiatives
Qualifications & Requirements
* A qualification in Occupational Therapy
* HCPC registration
* A full UK driving licence
* Access to a vehicle with business insurance
