We are recruiting a highly motivated, innovative and enthusiastic Specialist Pharmacist to join our forward thinking, friendly and expanding Health & Justice team at HMP High Down. By joining our Prisons Health Care Team, you will look after the healthcare needs of patients residing in secure environments and have the opportunity to help promote health and well-being. Our motto is "Caring NOT Judging" and we work in partnership with the criminal justice system and other agencies to provide responsive, dependable and high-quality primary care and mental health services to those who may have offended and those at risk of offending, to improve health status and encourage social inclusion.


Responsibilities

* To assist in delivering a clinical medicines management service to patients.
* To provide expert advice on pharmaceutical matters relating to primary care, substance misuse and mental health services, including risk management and ensuring compliance with medicines legislation and local medicines management policies.
* To provide input into policy and procedures affecting the use of pharmaceuticals at local and trust wide levels under the direction of the lead pharmacist.
* To liaise with dispensing pharmacy staff to support an effective dispensing and supply service. To participate in clinical audits and research associated with medicines and pharmacy practice.
* To deputise for the Lead Pharmacist in their absence.
* Ensure that all prescriptions and Medication Administration Records are clinically appropriate and meet necessary legal requirements.
* Help in the management of the repeat dispensing services ensuring full utilisation of this service.
* Run regular Medication review clinics for patients on the repeat dispensing service and, if appropriate, use prescribing skills to add and maintain patients onto this system.
* Ensure medicines reconciliation is completed for all patients on arrival into reception.
* Ensure safe and appropriate use of medicines to maximise benefit and minimise risk, including regular reviews of prescriptions, identification of significant drug interactions, advice for clinical monitoring, compliance with legal aspects (Medicines Act, Mental Health Act) and compliance with the Trust Formulary.
* Assist with the implementation of medicine related NICE publications, ensuring that recommendations from CNWL NHS Foundation Trust Medicines Management Group are considered and that systems for auditing and reporting on uptake are established.
* Provide clinical pharmacy input to the multidisciplinary team caring for patients and promote cost effective use of Medicines and other equipment.
* Promote the use of a preferred prescribing List (Formulary) and liaise with appropriate healthcare professionals to ensure cost effective use of clinically effective prescribed drugs.
* Ensure subordinate pharmacy staff providing a service to all prisons within the service line have adequate competencies to deliver such service.
* Provide a clinical pharmacy service to groups of patients, including reviewing prescriptions for accuracy and legality, ensuring medications are recorded in the electronic prescribing system (SystmOne), ensuring safe supply for discharge or court as applicable, monitoring outcomes and adverse effects, and recording all pharmacy interventions.
* Support the planning of patients release with respect to medication, including the implementation of a dispensing for discharge system and demonstrate professional accountability to patients.
* Provide advice to clinicians on unlicensed medicines use and areas of practice where the evidence base is lacking.
* Work across traditional boundaries as part of a fully integrated multidisciplinary team and actively participate in multidisciplinary committees to provide clinical pharmaceutical advice and develop disease management guidelines relevant to the prison population.
* Assist the lead pharmacist in the development of Direct Patient focussed Clinical Pharmacy Services, e.g., Pharmacy-Led Clinic services, Independent Prescribing, Medication issues directly to patient using PGDs, and Patient Medication Reviews.
* Manage and make appropriate referrals to other members of the multidisciplinary team and within the pharmacy department.
* Critically appraise literature to improve use of medicines and reduce risks by contributing to safe medication practice, including reporting medicines related errors on Datix.
* Develop and demonstrate expert clinical knowledge, motivate others, and act as a clinical role model to ensure safe, clinically effective and cost efficient use of medicines for junior staff.
* Provide pharmaceutical advice to health and associated staff as required, ensuring advice is accurate, evaluated, impartial and up-to-date within a prison setting.
