Job overview
The Clinical Practitioner in Urgent Community Response will act as a generalist ‘expert’ and exercise their clinical expertise in the long‑term conditions (LTCs) including frailty, levels of judgement, discretion and decision making in clinical care to deliver the Urgent Community Response Model.
Main duties of the job
Work closely with the wider multi‑disciplinary workforce in primary, community and secondary care to ensure patients receive appropriate investigation, intervention and treatment with minimal avoidable delays, whilst improving the quality of care in the right setting.
Work autonomously assessing individual patient needs, initiating investigations and initiating appropriate holistic, best‑evidence‑based treatment with the development of a personalised clinical management plan, including independent prescribing.
Ensure that the patient is placed at the centre of all decisions, reinforcing the concept of a streamlined patient journey, improving the quality of care.
Detailed job description and main responsibilities
* Develop staff knowledge and skills to promote equality and diversity and address inequalities, both in employment and service delivery. Ensure specific equality objectives are included in PDRs.

* Demonstrate a sound understanding of Clinical Governance and Risk Management and apply it to the work situation.
* Lead and participate in undertaking surveys and audits.
* Actively participate in clinical supervision and reflective practice with all members of the multi‑disciplinary team and on an individual basis.
* Ensure staff compliance with all mandatory training.
Qualifications / Training
* Registered Nurse – Level 1 (Adult) / AHP.
* Significant experience of a multidisciplinary approach to care.
* Evidence of up‑to‑date post‑registration education relevant to the role.
* Independent prescriber.
* MSc or equivalent level knowledge/experience in a relevant area.
Experience
* Significant experience as a registered health professional including working in a rehabilitation, frailty, community or older adult setting.
* Experience working as a registered nurse in the community.
* Significant experience of a multidisciplinary approach to care.
* Non‑medical prescribing.
* Nurse‑led triage.
* Nurse‑led management of minor illness, minor ailments and injuries.
* Mentoring or teaching qualification.
Skills / knowledge
* Advanced clinical practice skills; management of patients with long‑term conditions.
* Management of patients with complex needs.
* Clinical examination skills.
* Accountability for own role.
* Clinical leadership skills.
* Communication skills, both written and verbal.
* Communication of difficult messages to patients and families.

In accordance with UK Home Office requirements, Band 2 clinical and non‑clinical roles and Band 3 Non‑Clinical roles are not eligible for sponsorship.
Please note that the majority of non‑clinical roles (Agenda for Change Band 3 - 6) with the exception of those which appear on this list - link, will not meet the minimum requirements set by UK Visas and Immigration to sponsor candidates to work in the UK.
All non‑clinical roles (Agenda for Change Bands 7 - 9) that meet the prescribed skill level (RQF 6) and salary threshold will be eligible for sponsorship.

DBS Costs
Please be aware that all new employees starting work with the Trust will be charged for the cost of their DBS check if it is required for the role. The cost will be deducted from your salary in 3 payments over a 3‑month period once you start employment. By applying for this role, you are agreeing to these deductions being made. The charges for the DBS include the administration fee. The total costs are £26.40 for a standard check and £54.40 for an enhanced check. The level of check will be determined by the post you are applying for. For bank workers the full cost of the DBS check (as detailed above) will be deducted from your pay following completion of your online mandatory training. No payment will be made for your online training until it is all complete and verified by the Temporary Staffing Team. Please note: You will not be able to work any bank shifts until you have completed your mandatory training, and your DBS is in place.
