Job overview The Prostate Cancer Support Worker provides essential support to men affected by prostate cancer and their families throughout their care pathway. Working under the supervision of the Urology Clinical Nurse Specialist and as part of the multidisciplinary team (MDT), the post holder will ensure that patients receive high-quality, person-centred care from diagnosis through treatment and follow-up. The role focuses on supporting facilitating communication between patients and professionals, and ensuring timely access to clinical and supportive services. A confident understanding to the impact of cancer on individuals and families is needed as the role requires direct contact daily via phone or face to face during a worrying and difficult time for this group of people. You will be an advocate to the patient experience by independently managing holistic needs assessments, document patient information and follow-up on actions. The ideal applicant has knowledge of cancer care pathways and experience in a patient support, care coordination, or navigator in a health or social care setting. Possessing confident IT skills with ability to interact effectively with health professionals to benefit collaboration and excellent service provision is vital. Advert Act as a key point of contact for men diagnosed with or living with prostate cancer and their families, providing non-clinical information, advice and emotional support, escalating clinical concerns appropriately to the CNS or medical team. Undertake and document Holistic Needs Assessments (HNAs) and signposting patients to relevant support service. Maintain accurate and up-to-date patient records and databases and attention to track patients through the diagnostic and treatment pathway to prevent delays. Support the CNS team in the delivery of nurse-led clinics, patient education sessions, and health promotion activities. Participate in audits, data collection, and service evaluation projects related to prostate cancer care. Contribute to the ongoing improvement of patient pathways and experience within cancer services sharing time to the Single Point of Contact rota for all tumour groups with other CSW colleagues. Involved in risk stratification and delivery of open access patient follow up for prostate cancer patients which includes requesting surveillance bloods and facilitating timely imaging. The post holder will be based within the urology or oncology department and may work across outpatient clinics, inpatient areas, and administrative offices. Some flexibility in working hours may be required to meet service needs. Full training and ongoing supervision will be provided. We are unable to support sponsorship for this role. Coordinate the follow up investigations required post cancer diagnosis or treatment and provide a point of contact for patients post cancer treatment. Ability to promote patient empowerment and self-management strategies. Deliver self-management advice, support and know when to escalate to clinical team. Support and coordinate Holistic needs assessments for patients and completing personalised care and support planning, ensuring the monitoring patient needs. Responsible for coordinating rapid re-entry for specialist review against a clinically approved triage model. Provide fast and accurate clinical correspondence administration service with proficient use of appropriate software and systems to meet the required Trust’s standard of work. To coordinate and organise appointments and information. Arrange face to face and telephone communication and engagement with cancer patients and families. Ability to deal with level 1 distress and recognise and escalate concerns regarding psychological wellbeing and management. Involvement in the planning / organisation and delivery of health and wellbeing events and supported management events. Development of survivorship and wellbeing services. To support the department and organisation by carrying out any other duties that reasonably fit within the broad scope of a job of this grade and type of work.
