Health Care Assistant

As a Band 3 Health Care Assistant within our Bladder and Bowel Service, you will play a key role in supporting patients to improve their independence and overall quality of life in a community setting. Working closely with our specialist nurses, you’ll provide practical support, education, and reassurance, helping patients and carers feel confident in managing their care.
This is a rewarding role where you’ll build meaningful relationships with patients and contribute to delivering high‑quality, person‑centred care. It would suit someone who is compassionate, motivated, and able to work both independently and as part of a supportive team, with a genuine interest in making a difference to people's day‑to‑day lives.
Main duties of the job
Maintain high standards of patient‑centred care while supporting individuals with continence needs in a community setting. Undertake clinical tasks such as bladder scanning, sheath fitting, and contributing to product reviews, while monitoring outcomes and promoting independence. May be asked to run annual review clinics for patients in receipt of products and complete unscheduled visits, including urgent bladder scans, in response to clinical need. Work both autonomously and as part of a specialist team, using strong communication, organisational skills, and compassion to deliver safe, effective care and enhance patient wellbeing.

Job responsibilities
Job Purpose:
To assist the Continence Advisors and Continence Nurse in the provision of continence care and support of patients and families in the community setting.
Duties and Responsibilities
To work as part of the Bladder and Bowel Care Service providing specific care, including psychological and emotional support to patients in the community setting including nursing and residential homes, as written in care pathways.
To have the ability to work autonomously.
To undertake reviews of patients receiving continence supplies and assist monitoring the use of products.
To be responsible for the ordering of continence products, or aids. Monitor stock control of equipment required.
To work in accordance with Trust Policies and Procedures.
To be able to use a computer.
Clinical
To monitor skin integrity of patients at risk of pressure sores formation and provide basic preventative information.
To provide Health Promotion advice for patients e.g. monitoring diet and fluid intake, demonstration of products and aids.
To document all information and report findings to the continence advisors/continence nurse in accordance with Trust policies.
To provide clinical intervention for care of patients as directed by continence advisors/continence nurse following training and assessment of competencies e.g. Urinalysis.
To check equipment on a regular basis and ensure it is safe to use.
To report any Health and Safety or Infection Control issues to nurse specialist /advisor.
To assist in clinical practice as expected up to NVQ level 3.
Education and Development
Develop own clinical knowledge and identify areas for development within Personal Development Plan in collaboration with nurse specialist/ advisor.
Participate in the development of others clinical practice and knowledge.
To attend mandatory training in accordance with Trust Policies and Procedures.
Person Specification
Experience
* Demonstrable community experience.
* Ability to demonstrate basic continence knowledge through practice.
* Previous caring role.
* Training in aspects of continence care.
Relationships
* If you are related to a director or have a relationship with a director or employee of an appointing organization, please state the relationship.

Job Specific Requirements
* The ability to travel independently across the Trust.
* The ability to work flexibly in accordance with service needs.
Qualifications
* NVQ in Health care level 3 or equivalent experience.
