PCN Clinical Pharmacist at NHS – Join our friendly and innovative PCN in Wetherby, near Leeds and Harrogate. This is a permanent full‑time position (Band 7‑8a) based at Wetherby Health Centre and Bramham Medical Centre.
Job Summary
Provide specialist pharmaceutical support within a multi‑disciplinary team to improve care and support safe and effective prescribing. The role includes patient‑facing activities (face‑to‑face or telephone) such as chronic disease management, repeat prescription management, and structured medication reviews, with a focus on patients with polypharmacy. Proactively participate in quality improvement, clinical audits, and the Quality and Outcomes Framework to deliver an excellent service within general practice.
Key Responsibilities
* Conduct clinical medication reviews for patients both in person and via telephone across single or multiple long‑term conditions where medicines optimisation is required.
* Reconcile medicines following discharge from hospitals or intermediate care, working with patients and community pharmacists to ensure continuity of care and reduce readmission risk.
* Manage repeat prescription requests, resolve queries, and ensure appropriate monitoring tests are in place.
* Implement changes to medicines in line with MHRA alerts, product withdrawals, shortages, and local or national guidance.
* Take an active role in audit and quality improvement, implementing recommendations where appropriate.
* Provide independent patient information regarding medicines and prescribing changes, initiating further support from other healthcare professionals as needed.
* Identify clinical risk areas at network level and recommend new working practices to optimise prescribing quality and resource utilisation.
* Collaborate with the practice team to ensure full compliance with Care Quality Commission standards for safe and effective care.
Qualifications
* Master of Pharmacy (MPharm) degree, accredited by the GPhC with 1 year pre‑registration training.
* Demonstrated understanding of relevant professional standards and commitment to continuous professional development.
* Knowledge of IT systems (Word, email, internet) and clinical documentation tools (coding, template usage).
Required Experience
* Experience in primary care or general practice settings.
* Proficiency in conducting medication reviews, managing repeat prescriptions, and applying evidence‑based prescribing principles.
* Experience working with multidisciplinary teams and participating in audit/quality improvement projects.
* Capacity to influence and motivate stakeholders to adopt best practices.
Desirable
* Non‑medical prescriber status.
* Membership of the Primary Care Pharmacy Association (PCPA) or Royal Pharmaceutical Society.
* Experience with dispensing practice software and formulary management.
Other Requirements
* DBS reference standards and clear criminal record.
* Self‑motivation, adaptability, team‑working, safeguarding training.
* Access to private transport and ability to travel within the locality.
