An exciting opportunity has arisen for an Advanced Clinical Practitioner to work within Acute Frailty. The post holder will work across Urgent Care and Elderly Medicine directorates.
We require an enthusiastic, forward thinking, well organised and motivated individual with a positive attitude.
The post holder needs to hold an MSc in Advanced Practice or equivalent (health related studies), with core advanced practice competencies. Independent prescribing is a must.
Trainee ACP with experience in frailty and in their final year of training and near qualifying are encouraged to apply.
Main duties of the job
The Advanced Clinical Practitioner (ACP) will use specialist knowledge and skills to provide healthcare independently to patients. The ACP is accountable for the total episode of care for patients with undifferentiated and undiagnosed needs.
The post holder will act as a role model, delivering advanced practice and promoting clinical effectiveness in care. He/she will be required to work autonomously in delivering independent assessment, clinical examination, diagnosis, clinical decision making and planning of care and treatment, diagnosis, prescribing, health promotion and safe discharge of patients.
