This is an exciting opportunity to work as part of the new, expanded Health & Justice Service as a lead for the Liaison & Diversion Pathway within Worcestershire and Herefordshire. There will be a requirement to travel West Mercia wide on occasions to support the West Mercia Health & Justice Service.
This post will increase the capacity of the existing Health & Justice team and will be working closely with the current staff.
This role will include leading a team in providing high quality assessments for people in a Police Custody, Court and Community setting. The Liaison and Diversion service will provide a service to people who present with any vulnerability including but not limited to employment, housing, substance use, mental health, physical health, learning disability, head injury etc within the Criminal Justice System.
The successful applicants must be experienced in risk assessment, highly motivated and someone who is looking for a new challenge and have the skills to make a difference.
Due to the nature of the work posts will be subject to Police vetting.
Main duties of the job
Delivering a high-quality clinical service to patients and their families.
Managing complex assessment in the community and criminal justice establishments.
Responsible for clinical and caseload supervision of other team members where identified.
Diverting and liaising with multiple criminal police and other partners.
Responsible for participating in the setting of quality standards, including the auditing, monitoring, and reviewing of practice in line with current clinical guidance practice and policy.
Assess patient's individual care needs, agree, and refer on to appropriate agency.
Complete and contribute, where appropriate to robust risk assessments and risk management plans.
Deliver and promote positive risk taking, where appropriate, to improve quality of life and maximise occupational performance/independence.
Maintain an effective reporting system by observing and reporting verbally and in writing on patient condition.
Utilise agreed outcome measures to review the effectiveness of process, as part of Health & Justice Services.
Participate in the development of the service to ensure clinical governance processes are actively implemented at a team level.
Maintain an up-to-date level of professional competence.

The post holder will be responsible for ensuring that the resources within Herefordshire and Worcestershire L& D Service are maximised and will also have responsibility for the day to day and strategic management of the L& D Model.
The post holder will provide leadership, professional supervision & expertise when assessing vulnerabilities / complex needs within the Criminal Justice System including expertise of the Mental Health Act (Civil / Forensic Sections).
There is an expectation that all work is underpinned by theoretical and practice knowledge, and delivered to a high standard in line with statutory procedures and professional frameworks.
All aspects of the post may involve moderate physical effort: it will involve frequent mental effort with exposure to highly emotional circumstances and processing complex and contentious information in a sensitive manner.
The post holder will be responsible for providing advice / support to L& D Practitioners. Where required, post holder will be required to complete specialist assessment and interventions to people with a diverse range of needs coming into contact with the Criminal Justice System via the Liaison and Diversion service. The post holder will also be responsible for supporting / advising colleagues regarding complex needs.
The post holder will also be expected to be available for consultation, support and clinical supervision to other health and social care professionals working within Liaison and Diversion Team.
