An exciting opportunity has arisen for an experienced Advanced Clinical Practitioner to join an innovative and evolving community service delivering high-quality, person-centred care closer to home. This role is central to a programme of community redesign, focused on improving outcomes through proactive, integrated care. Working autonomously within your scope of practice, you will assess, diagnose, treat, and manage patients with complex and often undifferentiated conditions. Using advanced clinical reasoning, you will support independence, reduce unnecessary hospital admissions, and promote positive health outcomes. You will be a key member of a multidisciplinary team, working collaboratively with GPs, community nursing teams, geriatricians, allied health professionals, and wider partners to deliver coordinated, holistic care. As a senior clinician, you will provide clinical leadership, support service development, and contribute to innovative pathways and quality improvement initiatives. You will help ensure care is safe, effective, and aligned with evidence-based practice and national standards. A strong focus on patient experience, safety, infection prevention, and continuous development is essential, alongside the ability to build trusting relationships with patients, carers, and colleagues. Responsibilities
- Assess, diagnose, instigate appropriate investigations, plan, implement and evaluate treatment/interventions and care for patients presenting with an undifferentiated diagnosis.
- Clinically examine and assess patient needs from a physiological and psychological perspective, and plan clinical care accordingly.
- Assess, diagnose, plan, implement and evaluate interventions/treatments for patients with complex needs.
- Proactively identify, diagnose, and manage treatment plans for patients at risk of developing a long-term condition (as appropriate).
- Diagnose and manage both acute and chronic conditions, integrating both drug and non‑drug-based treatment methods into a management plan.
- Prescribe and review medication for therapeutic effectiveness, appropriate to patient needs in accordance with evidence-based practice and national and practice protocols, and within scope of practice.
- Work with patients to support compliance with and adherence to prescribed treatments. Provide information and advice on prescribed or over‑the‑counter medication regimes, side effects and interactions.
- Prioritise health problems and intervene appropriately to assist the patient in complex, urgent or emergency situations, including initiation of effective emergency care.
- Support patients to adopt health promotion strategies that promote healthy lifestyles and apply principles of self‑care.
- Demonstrate effective and compassionate communication and leadership skills to optimise impact.
