Advanced Clinical Practitioners (Prescriber)

We are seeking 2 dynamic and autonomous Advanced Clinical Practitioners (ACP) with independent prescribing qualifications to join our progressive and multidisciplinary Primary Care Network in North Wiltshire.

As an integral member of our patient-focused team, you will manage a caseload across a variety of clinical areas, providing advanced assessment, diagnosis, treatment, and evaluation. You will work collaboratively with GPs, pharmacists, paramedics, care coordinators and health & well-being coach to deliver responsive, high-quality care to our population of over 54,000 residents.

The role demands a high level of clinical competence, critical decision-making, and the ability to work independently while contributing to holistic patient care and service transformation.

Main duties of the job

* Seeing urgent/same day appointmentsas part of the multidisciplinary duty team
* Ward round and care home visit
* Provide advanced clinicalassessment, diagnosis, treatment, and management of patients across a range ofconditions.
* Work autonomously and as part of amultidisciplinary team to deliver high-quality care.
* Support the development andimplementation of clinical pathways and service improvements.
* Contribute to the supervision andmentoring of junior staff and trainees
* Engage in audit, qualityimprovement, and professional development activities
* Management of long-term conditionsboth the physical and psychosocial needs of patients with strategies forbehavioural change and motivational counselling
* Covid management clinical oversight

Job responsibilities

The following are the coreresponsibilities of the role. There may be on occasion a requirement to carryout other tasks; this will be dependent upon factors such as workload andstaffing levels:

a)Work as part of the acute / on the day team to manageincoming demand in the practices and, at times, by home visiting or coveringward round.

b)Appropriately assess, examine, investigate, diagnose andtreat/advise a wide range of on-the-day presentations be these minor illnessesor exacerbations of chronic conditions as part of our acute provision.

c)To be professionally and legally accountable for all workundertaken. To practice at an advanced level of professional autonomy andaccountability that is within the Professional Codes of Conduct

d)Requestsinvestigations such as blood, urine and other laboratory tests,electrocardiographs (ECGs), ultrasound scans, X-Rays and computed tomography(CT) scans in accordance with IR(ME)R regulations and as local policy allows.

e)Assesses patients with mental health needs using localpolicy and guidelines and refers to the appropriate services.

f)Ensure patients are informed and consent is gained priorto treatment, investigation and management, as appropriate

g)Take responsibility for the management of patients withinown caseload and supervision/development of junior members of themulti-professional team.

h)Identify the need for appropriate diagnostic tests andinterpret the results if appropriate

i)Identify and intervene proactively where circumstancescontribute to an unsafe environment for patients, staff or relatives andescalate these appropriately

j)Share knowledge and expertise, acting as an expertresource to others

k)Support and liaise with the wider MDT as needed

l)Contribute to the development and improvement of systemsand processes that facilitate patients being able to stay in their own homes

m)To improve the quality of the patient experience byidentifying and meeting the individual clinical needs of patients, includingissues pertaining to child protection and vulnerable adults.

n) Process and interpret pathology andother test results as required (within the scope of practice)

o) Maintain accurate clinical records and ensureread codes are used effectively

p) Support patients in the use of theirprescribed medicines or over the counter medicines (within own scope ofpractice)

q) Contribute to practice targets (QOFetc.), complying with local and regional guidance

r) Liaise with external services / agenciesto ensure the patient is supported appropriately (vulnerable patients etc.)

s) Support the clinical team with allsafeguarding matters, in accordance with local and national policies

t) Understand practice and local policiesfor substance abuse and addictive behaviour, referring patients appropriately

u) Autonomously formulates appropriatemanagement plans for patients, formulates clinical decisions and often complextreatment plans to manage acute illness and acute episodes of chronic illnessincluding referral, admission, or discharge from service.

v) Prioritises health problems andintervenes appropriately in complex, urgent and emergency situations, includinginitiation and leadership of resuscitation.

w) Manages clinical events involvingpatients, often requiring unpredictable and high levels of mental and/orphysical effort according to the patients dependency and clinical need.

y)Covid management clinical oversight.


Person Specification


Experience

* Significant experience at senior level, Band 7 or above
* Evidence of teaching in a variety of environments
* Experience of working to protocols and guidelines
* Evidence of on-going personal and professional development
* Previous experience of developing clinical guidelines and clinical care pathways
* Experience of clinical audit
* Evidence of successful management of change management
* Previous experience in an ACP role


Qualifications

* Registered with the appropriate UK statutory regulatory body for your profession
* Post graduate certificate or diploma (Advanced Practice Qualification)
* Registered as an independent prescriber
* Long term conditions qualification
